摘要
目的评估慢性阻塞性肺病(COPD)人群中代谢综合征的发生率,并探索两者间潜在的关联。方法老年病科的呼吸门诊中征募70例男性COPD患者及68例男性吸烟非COPD患者分别作为COPD组和对照组。每例患者进行腰围、胆固醇、甘油三脂、高密度脂蛋白、低密度脂蛋白、空腹血糖、血压及肺功能测定,采用国际糖尿病联盟的定义诊断代谢综合征。结果COPD组有30名(42.9%)患代谢综合征,明显高于对照组(22.1%)(P<0.05)。代谢综合征的5个组分中只有腹型肥胖、血压升高和血糖升高在COPD组的发生率高于对照组,而COPD组的低HDL-C和高TG发生率低于对照组。重度、极重度COPD患者的代谢综合征发病率较轻中度患者升高。结论本老年病科的呼吸门诊中,代谢综合征在男性COPD患者中相当普遍,可能是COPD患者增加患心血管疾病风险的一个原因,在此人群中进行代谢综合征的检查是必要的。
Objective To evaluate the prevalence of metabolic syndrome in patients with chronic obstructive pulmonary disease (COPD) and to explore the potential association between them. Methods Seventy COPD patients and 68 non-COPD patients were recruited from the respiratory outpatients of the Department of Geriatrics in Ruijin Hospital, all of whom are male and had a history of smoking. Waist circumference, cholesterol, triglycerides, high density lipoprotein cholesterol, low density lipoprotein cholesterol, fasting glucose, blood pressure and lung function were ascertained for each patient .The metabolic syndrome was diagnosed according to the International Diabetes Federation Worldwide Definition. Results Thirty patients with COPD (42.9%) presented metabolic syndrome, significantly higher than the control participants (22.1%) (P 〈 0.05). The prevalence of abdominal obesity, raised blood pressure and elevated fasting glucose in patients with COPD was higher than that in control participants. The severe and very severe COPD patients had higher prevalence of metabolic syndrome than mild and moderate ones. Conclusion The metabolic syndrome is prevalent in the patients with COPD among the respiratory outpatients in the Department of Geriatrics. It might be an explanation for the increased risk of cardiovascular disease in patients with COPD. Screening for the metabolic syndrome among this population is well demanded.
