摘要
目的:探讨特制型人工肱骨头置换治疗肱骨上端骨肿瘤的疗效。方法:采用特制型人工肱骨头置换柄治疗在本科住院的肱骨上端骨肿瘤患者9例。患者均为单侧病损,使用特制型非限制性人工肱骨头假体置换,用骨水泥固定于肱骨残端髓腔内,外缝合固定一层塔夫纶套,然后把关节囊、肌腱和肌肉缝合固定在其上,以重建肌附着。结果:所有9例患者皆得以随访,时间为6月-3年,其功能良好,优良率达89%,术后无1例感染及脱位。结论:肱骨上端骨肿瘤治疗要严格掌握适应证,采用特制型个体化人工肱骨头结合塔夫纶套骨膜进行置换具有良好的效果,对保全肢体、提高患者生存质量是一种疗效可靠的手术方法。
Objective：To investigate the outcome of patient-tailored prosthesis for humeral head replacement in upper humeral bone tumors. Methods： nine cases of upper humeral bone tumor admitted to our hospital were treated. All of these unilateral lesions were removed, followed by humeral head replacement with patient-tailored non-restrictive prosthesis. The end of prosthesis was inserted into marrow cavity of the humeral stump and secured with bone cement. A Teflon cuff was then sutured to fix around the site. Reconstruction of muscle insertion was accomplished by stitching the articular capsule, tendons and muscles onto the Teflon cuff. Results： During 0.5 to 3 years＇ follow-up, 89% of the patients showed good recovery of functions. None had dislocation or infection after operation. Conclusion： when strictly controlled for indications, the use of patient-tailored prosthesis combined with Teflon-cuff periosteum for upper humeral tumors appeared to be reliable and safe for limb salvage and improvement of life quality.
