多层螺旋CT三维血管成像对颅内动脉瘤诊断价值的研究 被引量:21

摘要 目的评估多层螺旋CT三维血管成像(MSCTA)在诊断颅内动脉瘤的价值。资料与方法对临床怀疑为颅内动脉瘤的73例患者的影像资料进行回顾性分析,其中23例行MSCTA检查,36例行DSA检查,12例同时行MSCTA和DSA检查,2例行MSCTA检查的同时与外科手术对照。结果MSCTA和DSA的可信度平均得分分别为7.87分和8.50分(P<0.05);MSCTA对颅内动脉瘤诊断的敏感性、特异性和准确性分别为92%、80%和91%。结论尽管MSCTA在颅内动脉瘤的诊断中可信度略低于DSA,但其敏感性、特异性和准确性较高,而且MSCTA通过三维后处理方法能很好地显示颅内动脉瘤的特征和周围的解剖结构,对颅内动脉瘤的诊断、外科手术有着重要的意义。 Objective To assess the diagnostic value of three dimensional (3D) multi slice spiral computed tomographic angiography (MSCTA) in detecting intraeranial aneurysms. Materials and Methods The imaging data of 73 consecutive patients suspected with intracranial aneurysms were retrospectively analyzed, 23 patients were performed with MSCTA. 36 patients with DSA, 12 patients with both MSCTA and DSA, 2 patients with MSCTA, results of MSCTA were compared with operation. Results Analysis of the mean confidence scores showed that there was slightly less confidential in MSCTA than that in DSA groups (7.87 vs 8.50 P 〈 0.05). The sensitivity, specificity and accuracy for detection of aneurysms with MSCTA were 93% , 80% and 91% respectively. Conclusion MSCTA has high sensitivity, specificity and accuracy for detection of intracranial aneurysms although there was slightly less confidence in MSCTA than that in DSA. However, MSCTA has the ability to accurately characterize aneurysms and delineate anatomic landmarks such as adjacent branch vessels and bones using 3D postprocessing methods, which has important significance in the diagnosis and operation for intracranial aneurysms.
