摘要 目的:分析32例胃肠道间质瘤(GISTs)病人组织切片中p53、Ki-67、Bcl-2和CyclinD1表达及其与预后的相关性。方法:采用免疫组化SABC法检测32例GISTs患者p53、Ki-67、Bcl-2和CyclinD1的表达。结果:p53在良性组均呈阴性,在交界组与恶性组呈阳性者为3例和15例;Ki-67指数在三组中表达分别为(8.97±4.12)%、(16.21±7.44)%和(22.33±9.14)%(P<0.01);Bcl-2在三组呈阳性者分别为2例、3例和12例;CyclinD1指数在三组中表达分别为16.95%、51.22%和69.38%。Ki-67表达大于20%有12例,存活时间平均10.5月,Ki-67表达小于20%的存活时间平均22.4月。结论:Ki-67增值指数不仅可用作评价GISTs恶性潜能的指标,而且Ki-67指数大于20%具有独立的预后意义。 Objective: To explore the expression of p53、Ki-67、Bcl-2 and CyclinD1 in gastrointestinal stromal tumors (GISTs) and their correlation with malignant tumor prognosis.Method: Immunohistochemical technique SABC method was used to detect the expression of p53、Ki-67、Bcl-2 and CyclinD1 in 32 cases of GISTs. Results: The positive rate of p53 in the benign, the borderline and the malignant GISTs was 0, 9.38% and 46.88% respectively. The Ki-67 labeling index in the benign, the borderline and the malignant GISTs was (8.97±4.12)%,(16.21±7.44)% and (22.33±9.14)% respectively, showing a significant difference among the three groups(P<0.01). The positive rate of Bcl-2 in the benign, the borderline and the malignant GISTs was 6.25%,9.38% and 37.50% respectively, and the positive rate of CyclinD1 was 16.95%, 51.22% and 69.38% respectively( P<0.05).There were 12 cases expressed over 20% of Ki-67,and their mean survival time was 10.5 months.The mean sumval time of cases with Ki-67 expression less than 20% was 22.4 months.Conclusion: The Ki-67 protein is a valuable biomarker in the differential diagnosis among the benign, the borderline and the malignant GISTs.The labeling index over 20% is an importent prognostic factor for GISTs.
