摘要
目的 了解我院院内感染常见病原菌构成及药物敏感状况。方法 采用半自动微生物分析仪(ATB)对细菌鉴定及药物敏感情况进行分析。结果 2005年我院院内感染前六位病原菌依次为大肠杆菌35株(23.3%)、金黄色葡萄球菌21株(14.0%)、肺炎克雷伯菌21株(14.0%)、铜绿假单胞菌20株(13.3%)、粪肠球菌10株(6.7%)、阴沟肠杆菌8株(5.3%),以上细菌占总检出率的76.7%,其余各类细菌占23.3%。革兰氏阳性球菌敏感药物依次为万古霉素、替考拉宁、利福平、磺胺甲基异哮唑。革兰氏阴性杆菌敏感药物依次为泰能、头孢吡肟、头孢他啶、阿米卡星。产ESBLs菌株发生率12.7%,为大肠杆菌13株(68.8%)、肺炎克雷伯菌6株(31.2%),均对泰能敏感。结论 正常菌群和条件致病菌正成为感染谱中的主要病原菌,万古霉素对革兰氏阳性球菌敏感;泰能、头孢吡肟对革兰氏阴性杆菌仍有较高敏感性。
Objective To understand the pathogen composition and drug susceptibility of nosocomial infection in our hospital. Methods The bacteria were identified and the drug-susceptibility tests were performed with ATB microbial half-auto-analyzer. Results The top six species were Escherichia coli 35(23.3%), Staphylococcus aureus 21(14.0%), Klebsiella pneumoniae 21(14.0%), Pseudomonas aeruginosa 20(13.3%), Enterococcus fecalis 10(6.7%), and Enterobacter cloacae 8(5.3%). Gram-positive bacteria were susceptible to Vancomycin, Timoplanin, Rifampin and Sulfamethoxazole. Gram-negative bacteria were susceptible to Tienam, Cefepime, Ceftazidime and Amikacin. The average positive rate of ESBLs positive bacteria was 12.7%, among them, the ratio of Escherichia and Klebsiella pneumoniae were 68.8% (13) and 31.2% (6). They were susceptible to Tienam. Conclusions The normal bacteria flora and conditional pathogens are becoming main pathogens of the infectious diseases. But Gram-positive bacteria were susceptible to Vancomycin. Gram-negative bacteria were higher susceptible to Tienam, Cefepime.
