摘要
目的:探讨床旁经纤维支气管镜(简称纤支镜)引导下的鼻腔气管插管机械通气的护理效果。方法:将连续收治的139例患者,随机分为经鼻腔气管插管(A)组和经口腔气管插管(B)组。A组为纤支镜引导下的鼻腔气管插管,B组为按常规口腔气管插管。结果:通气前后组内比较,各项血气监测指标均有改善(P<0.05)。组间比较没有差异性(P>0.05)。A组发生呼吸机相关性肺炎(VAP)16例(23.19%),B组发生VAP27例(占38.57%)。A组明显低于B组(P<0.05)。因VAP致死亡的A组13例(19%),B组23例(占34.2%);机械通气平均时间A组为119.234±45.432h,B组为136.768±56.346h,A组较B组明显缩短(P<0.05)。结论:对需机械通气的患者,尽量采取经纤支镜引导下的鼻腔气管插管,有助于缩短机械通气时间,降低VAP的发生率。
Objective: Study the nursing result of mechanic airing by putting a tube through the nose led by a bed -side fabric bronchus mirror (simply called fabric bronchus mirror ). Methods: Divide the successive received 139 patients into two groups randomly. One is putting a tube through the nose led by a fabric bronchus mirror (group A ) and the other is putting a tube through the mouth normally ( group B ). Restilt: Compare within the groups before and after the airing. All the measurable blood and air indexes have improved ( the average P 〈 0.05 ). There is no difference in comparison between the groups ( P 〉 0.05 ). There is 16 VAP cases (23.19%) taking placing in group A, and there is 27 VAP cases taking placing in group B (38.57%). Group A is clearly lower than group B (P〈0.05). 13 patients in group A died of VAP ( 19% ) and 23 patients in group B died of VAP ( 34.2% ). The average airing time in group A is 119. 234 ±45.432 hours, and the average airing time in group B is 136;768 ±56.346 hours. Group A is clearly shortened compared with group B ( P 〈 0.05 ). Conclusion: To the patients who need mechanic airing, the method of putting a tube through the nose led by a fabric bronchus mirror can help shorten the mechanic airing time and reduce VAP happenings.
