摘要
目的评价动态心电图(DCG)对心房按需型(AAI)、心室按需型(VVI)、双腔按需型(DDD)起搏功能异常判断的价值。方法利用DCG监测导联CM1、CM3及CM5,应用回放式计算机分析系统,分析起搏与感知功能。结果159例患者经DCG检出VVI起搏者14例伴起搏、感知功能障碍,DDD起搏者2例伴起搏、感知功能障碍,1例伪感知功能障碍。结论DCG自动分析系统对VVI起搏模式的ECG自动检出正确率最高,对AAI起搏模式次之,AAI通过人工识别后仍可取得满意效果;但对DDD起搏模式的DCG分析则最困难。
Objective To assess the values of dynamic electrocardiography(DCG) in the diagnosis of AAI,VVI and DDD pacing dysfunction. Methods By means of DCG monitoring leads(CM1, CM3 ,CM5) and playback computer analysis system, pacing function of pacemakers was analyzed. Results Of 159 patients, 14 VVI patients were diagnosed as pacing and sensing dysfunction, 2 DDD patients were diagnosed as pacing and sensing dysfunction and 1 as sensing dysfunction. Conclusion automatic analysis system provides most accuracy in ECG of VVI pacing mode, less in AAI pacing mode and the least in DDD pacing mode. After artificial identification, DCG remained to be a diagnostic manner in AAI pacing modes.
