摘要
目的探讨基质金属蛋白酶2(MMP2)和CA50在大肠癌、癌旁黏膜和正常大肠黏膜中的表达及临床意义。方法用免疫组织化学法检测40例大肠癌组织、癌近旁黏膜组织、癌远旁黏膜组织及10例正常大肠黏膜组织中MMP2和CA50的表达。结果①MMP2和CA50的阳性表达强度及表达阳性率按正常大肠黏膜组织、远癌旁黏膜组织、近癌旁黏膜组织及大肠癌组织的顺序依次增高,且表达强度和阳性率在4种不同组织间的差异均有统计学意义(P<0.01);②大肠癌组织中MMP2表达和CA50表达有一定关系;③CA50表达与大肠癌的分化程度有关(P=0.02);MMP2表达与大肠癌的分化程度及Dukes分期有关(P=0.04,P=0.02)。结论MMP2的过度表达可促进大肠癌的恶性发展,CA50是评估大肠癌细胞恶性程度的可靠指标,MMP2和CA50在大肠癌的发生、发展过程中可能有协同作用。
Objective To study the expressions of matrix metalloproteinase 2 （MMP2） and carbohydrate antigen 50 （CA50） in colorectal carcinoma, cancer-adjacent mucosa （2 cm from the nether edge of tumor）, cancer-distant mucosa （5 cm from the nether edge of tumor） and normal colorectal mucosa, and to elucidate their effects on the development of colorectal carcinoma. Methods The expressions of MMP2 and CA50 were detected immunohistochemically in 40 cases of colorectal carcinoma, cancer-adjacent mucosa, cancer-distant mucosa and 10 cases of normal colorectal mucosa. Results （1）The expression intensity and positive rates of MMP-2 and CA50 increased significantly in turn by normal mucosa, cancer-distant mucosa, cancer-adjacent mucosa and colorectal carcinoma. （2）The expression of MMP2 was correlated with CA50 in colorectal carcinoma. （3）The expression of CA50 in colorectal carci- noma was closely associated with tumor differentiation, and the expression of MMP2 in colorectal carcinoma was closely associated with differentiation and Dukes stages as well. Conclusion Over expression of MMP2 facilitates the malignant progress of colorectal carcinoma; CA50 is a reliable marker of malignance in colorectal carcinoma; CA50 and MMP2 may have synergetic effects on the development of colorectal carcinoma.
