摘要
目的探讨术中氩氦刀(argon super-cryosurgery system,ASCS)治疗中晚期肝癌的适应证并总结其相关并发症。方法回顾性分析术中 ASCS 治疗的45例中晚期肝癌病人的临床资料。结果成功治疗45例共47次,无手术死亡。术后影像学提示经治疗的癌肿均有不同程度液化、坏死,癌肿缩小1～13cm;甲胎蛋白(AFP)下降5.6～5931μg/L。与术中 ASCS 治疗相关的并发症主要有:出血3例、胸腔积液5例、胆汁漏3例、左心衰1例、轻度肝昏迷2例,经相应处理全部临床治愈。结论术中 ASCS 治疗为中晚期肝癌病人提供了新的有效治疗途径;必须注意选择适应证;加强围手术期处理能有效减少并发症。
Objective To investigate the indications and complications in treatment of advanced liver cancer by Argon super-cryosurgery system （ASCS）. Methods The clinical data of 45 patients with advanced liver cancer treated by ASCS in our hospital were retrospectively analyzed. Results A total of 47 times of the treatment were successfully performed in the 45 patients, Imaging revealed that the tumors shrank for 1 - 13 cm after the treatment. Meanwhile, necrotic and liquefacient changes were observed. AFP decreased for 5.6 - 5931μg/L. The complications included bleeding in 4 patients, bile leakage in 3, left heart failure in 1, thoracic effusion in 5 and liver cataphora in 2. Conclusions ASCS during operation offers a new and effective method for treatment of advanced liver cancer. Careful selection of indications and strengthening perioperative management are helpful to reduce complications.
