摘要
本研究主要观察暂时性阻断肝静脉下行肝动脉栓塞(THAE-THVO)后的病理改变。8只犬进行了THAE-THVO。治疗时在球囊导管完全阻断相应区域肝静脉下向肝动脉注入超液化碘油,然后注入明胶海绵碎片;对照组4只,仅做肝动脉栓塞(THAE)。术后观察1~8周,复查肝功能、CT,间隔1~2周处死动物行病理学检查。THAE-THVO与THAE后肝功能指标(AST,ALT)均呈一过性增高,二组间差别无显著性意义。术后3周CT检查,THAE-THVO组碘油呈大片状或近似肝段分布,部分门脉分支内有碘油沉积;对照组仅见散在小点碘油沉积。病理检查:THAE-THVO后坏死范围较大,呈楔状或扇形分布,肝窦、门静脉小分支及小动脉内可见碘油颗粒。对照组见散在灶性坏死。THAE-THVO后栓塞区坏死范围较大,门脉小分支内有碘油者比例较高。
To observe the pathologic changes after transcatheter hepatic arterial embolization under temporary hepatic venous occlusion (THAE-THVO). THAE-THVO was performed in 8 dogs. THAE -THVO was carried out by introducing a catheter into the lobe or segmental arteries and injecting of lipiodol followed by gelatin sponge particles, under a balloon catheter complete occlusion of the corresponding hepatic vein. THAE without hepatic vein occlusion was performed in 4 dogs as the control. Animals were followed up for 1~8 weeks with liver function test, CT and gross and microscopic examination.Transient changes in liver function was found in all dogs, there was no significant difference between THAE-THVO and THAE. On follow up CT, dense lipiodol accumulation was found in the embolized lobe after THAE-THVO, and a scattered punctate lipiodol accumulation was found in the control group. At pathologic examination after THAE-THVO,large area or segmental necrosis was found in the embolized lobe, lipiodol deposition could be seen in the hepatic sinus, small protal branches, and the arteriolae.In the control group,scattered foci necrosis was found in the embolized area. Temporary lobe or segmental hepatic venous occlusion can improve the results of transcatheter hepatic aterial chemoembolization for liver tumors.
