Arthroscopic treatment of old tibial eminence fractures

摘要 目的探讨关节镜辅助下陈旧性胫骨髁间棘骨折的治疗。方法对2001年1月-2004年12月10例陈旧性胫骨髁间棘骨折进行回顾性分析。成人骨折8例,儿童骨折2例,按Meyers—Mckeever骨折分型:Ⅱ型1例,Ⅲ型8例,ZaricznylⅣ型1例。固定方法:单枚空心拉力螺钉固定6例,交叉克氏针固定3例.其中2例为儿童,交叉韧带重建1例。随访时间:10~36个月。结果骨折平均临床愈合时间6周。术后3个月Lysholm膝关节功能评分:90.7±2.1,术后6个月评分:93.4±2.6.所有患膝恢复健侧活动度。结论陈旧性胫骨棘骨折多由新鲜骨折治疗不及时或治疗不当演变而来;对伴有轻度前叉韧带挛缩或松弛的骨折可以复位固定。 Objective To investigate the treatment of old tibial eminence fracture under arthroseopy. Methods From January 2001 to December 2004, 10 cases of old tibial eminence fractures were followed up. There were 8 cases in adults and 2 cases in children. According to Meyers-Mekeever classification systems, there were 1 case of type Ⅱ , 8 cases of type Ⅲ ,and 1 case of type ZarieznylIV. Six cases of adult were locked by a eannulated tensile screw, 1 case of adult and 2 cases of children locked by cross Kirsehner Wire, and 1 case combined with anterior erueiate ligament (ACL) injury reconstructed with bone-patellar tendon-bone (B- PT-B) allograft. All cases were followed up for 10-36 months. Results The average time of bone healing was 6 weeks. No knee laxity or instability was detected in all cases at 3 months postoperatively. At same time, full range of motion of the affected knee returned,and the average Lysholm knee score was 90.7±2.1. The average Lysholm knee score was 93.4±2.6 at 6 months postoperatively. The Laehman test and ADT test was negative. Conclusion The old fractures are developed from fresh fractures because of misdiagnosis or malpractice . The fractures combined with ACL light contraction or lack may acquire reliable reduction and fixation. Inversely, the anterior crueiate ligament should be reconstructed after injury completely.
