摘要
目的:探讨育龄妇女正常月经周期中部分脂代谢指标的变化,与卵巢功能变化及内生激素(E2)水平变化的相关性,以期望为相关疾病的临床诊断和治疗提供参考。方法:健康育龄妇女170例,(22~50)岁,将月经周期分为经期(MEN)、早卵泡期(EF)、晚卵泡期(LF)、黄体中期(LUT),检测雌激素(E2)、孕激素(P)、黄体生成素(LH)、卵泡刺激素(FSH)、脂质和脂蛋白。结果:LF高密度脂蛋白胆固醇(HDL-C)比MEN显著升高(P〈0.05),LUT低密度脂蛋白胆固醇(LDL-C)比EF显著降低(P〈0.05),LF时HDL-C/载脂蛋白AI(apoAI)比MEN升高10%;LUT期LDL—C/HDL—C比MEN降低4%、比EF降低7%;LUT期apoAI/载脂蛋白B(apoB)比MEN升高6%、比EF升高9%;LF甘油三脂(TG)比LUT升高5%,均未达到统计学显著性。年龄(29~49)岁标本60例,以月经周期第3d测定FSH水平作为衡量卵巢功能的指标,FSH≥7IU/L时,对比与FSH〈7IU/L组,LDL—C(P〈0.001)、TC(P〈0.001)、apoB(P〈0.005)、LDL—C/HDL—C(P〈0.05)均显著升高,雌激素两组间无显著差异。结论:育龄妇女正常月经周期中脂质和脂蛋白的代谢一定程度上受到内生激素的影响,但短暂的低雌激素状态并不明显影响脂代谢。卵巢的储备功能可能是评估绝经期前女性心血管疾病发生风险的另一个因素。
Objective To investigate the relationship between the changes of serum lipids and lipoproteins levels and changes of ovarian function (expressed as E2 levels) during normal menstrual cycle in women of child -bearing age. Methods Serum levels of E2, LH, P were measured with CLIA and serum contents of apolipoproteins A - I ( apoA - I), apolipoproteins B ( apoB), high - density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), total cholesterol (TC), triglyceride (TG), lipoprotein(a) [ LP(a) ] were measured with biochemical method in 170 healthy women of child - bearing age druing various phases of the menstrual cycle ( MEN, EF, LF, LUT). Besides, FSH levels were measured with CLIA in 60 women ( aged 29 - 49) on day 3 of the menstrual cycle. Results The following changes of the contents of lipid metabolism parameters were noted : ① In LF phase HDL - C concentrations were significantly higher than those in MEN phase (P 〈 0.05 ), ② LDL - C concentrations were significantly lower in LUT phase than those EF phase, ③ During LF, HDL = C/apo AI ratio was 10% higher than that during MEN, ④ During LUT, LDL - C/HDL - C ratio was 4% lower than that during MEN and 7% lower than tnat during ET, apo AI/apo B ratio was 6% higher than that during MEN and 9% higher than that during ET, TG contents were 5% lower than those during LF. However, these fluctuations were not statistically significantly. In 32 women with basal FSH concentration on menstrual cycle day 3 ( a useful biomarker to es- timate ovarian reserve) over 7IU/L, LDL - C (P 〈 0.001 ), TC ( P 〈 0. 001, apoB ( P 〈 0. 005 ), LDL - C/HDL - C ( P 〈 0.05 ) levels were significantly higher than those in women with FSH less than 7IU/L, but the E2 levels were not significantly different. Conclusion Endogenous estrogens do affect lipid and lipoprotein metabolism, but short - time lower E2 levels in certain phases of normal menstrual cycle does not persistently influenc
