摘要
目的:观察颅脑手术的病人行吸入异氟烷或全凭静脉麻醉颅内血糖及乳酸的变化。方法:40例颅内钟瘤切除术病人,ASAⅠ-Ⅱ级,随机分为异氟烷组(Ⅰ组)和丙泊酚组(Ⅱ组),每组20例。两组麻醉诱导用药相同,Ⅰ组采用异氟烷吸入维持麻醉,Ⅱ组采用丙泊酚静脉维持。于诱导前、插管后、麻醉开始20min、麻醉开始60min采颈内静脉血测定血糖和乳酸浓度。结果:Ⅱ组各时点血糖、乳酸浓度均无显著变化,Ⅰ组血糖浓度在20min及60min明显升高(P<0.05),乳酸浓度在麻醉后60min显著升高(P<0.05)结论:丙泊酚具有一定的脑保护作用,可以降低应激反应。
Objective:To study the changes of blood concentration of glucose and lactate during brian operation under TIVA or isoflurane anesthesisa.Methods:40 patients for brain operation were randomized to isoflurane group( Ⅰ ) and propolof group( Ⅱ )respectively(n = 20). Cerebral blood samples were taken for the measurements of blood glucose and lactate before induction, after intubation, at 20min and 60 min anesthetic maintenance respectively. Results: Blood concentrations of glucose and lactate were not significantly changes in propofol group.In isoflurane group,however, blood concentration of glucose significantly increased at 20 min and 60 min( P 〈 0.05), and lactate significantly increased at 60 imin( P 〈 0.05). Conclusion: propofol has a cerabral protective effection and decreased stress periopemtively during brain operation.
