摘要
[目的]探讨跟骰关节融合对足部运动的影响程度及其临床意义。[方法]选取10侧新鲜尸体足标本,通过建立非负重位尸体足模型结合力偶矩、弯矩及平衡加载方法,测量分析跟骰关节融合前后,大体足背屈跖屈、外展内收、外翻内翻极限运动度的变化。[结果]跟骰关节融合前后,除背屈极限运动度变化无统计学意义,其余各方向运动较术前明显减小(P<0.01),其中跖屈受限2.11%±4.66%、外展36.01%±4.13%、内收19.01%±4.27%、外翻48.14%±5.63%、内翻20.52%±4.28%。[结论]跟骰关节融合对大体足运动存在较大的限制作用,临床应用该术式时不可忽视此负面效应。
[ Objective] To discuss the effect of the calcaneocuboid arthrodesis on kinematics of foot and its clinical significance. [ Method] In10 fresh-frozen foot specimens,limitation dorsiflexion, plantoflexion, abduction, adduction , eversion, inversion motion of foot were determined before and after calcaneocuboid arthrodesis under non-weight with moment of couple, bending moment, equilibrium dynamic loading. [ Result] A significant decrease in the motion of foot was observed (P 〈 0.01 ) in our cadaver model following calcaneocuboid arthrodesis except the dorsiflexion of foot. Motion of foot was diminished by 2. 11% ± 4. 66% in plantoflexion; 36.01%±4. 13% in abduction; 19.01% ±4.27% in adduction; 48.1% ±5.63% in eversion; 20. 52% ± 4.28% in inversion. [ Conclusion ] Kinematics of foot limited obviously following calcaneocuboid arthrodesis, we must pay attention to it in clinical application.
