重组人表皮生长因子对女性面部Ⅱ度烧伤创面修复的临床观察 被引量:3

摘要 目的:观察外用重组人表皮生长因子(rhEGF)对面部Ⅱ度烧伤创面的治疗作用和安全性。方法:随机选择成年女性面部浅Ⅱ度烧伤、深Ⅱ度烧伤各60例,进行随机、双盲、同体试验;每例患者选择一块浅Ⅱ度或深Ⅱ度烧伤创面,并将其分为面积相近的两部分作为治疗组及对照组;伤后3 d起,治疗组采用含rhEGF的等渗盐水纱布覆盖创面,对照组单用等渗盐水纱布覆盖创面,每日换药1次至创面愈合。观察中期创面愈合率、愈合时间、疼痛情况及Ⅱ度创面愈合后的瘢痕指数。结果:对浅Ⅱ度烧伤创面,治疗组的7 d愈合率显著高于对照组[(77.18±13.36)%vs.(58.97±10.39)%,P<0.01],愈合时间显著短于对照组[(9.92±2.48)d vs.(12.07±3.05)d,P<0.01]。对深Ⅱ度烧伤创面,治疗组的14 d愈合率显著高于对照组[(75.93±22.02)%vs.(62.26±19.25)%,P<0.01],愈合时间显著短于对照组[(17.12±3.69)d vs.(21.06±3.87)d,P<0.01],瘢痕指数明显低于对照组[(8.12±1.47)vs.(9.79±1.85),P<0.01]。治疗过程未见明显不良反应。结论:外用rhEGF能显著加快面部Ⅱ度烧伤创面的愈合速度,缩短愈合时间,提高愈合质量,减少瘢痕的形成,无明显不良反应,安全性好。 Objective:To explore the therapeutic effect and security of recombinant human epidermal growth factor(rhEGF) for the facial burn wounds of degree Ⅱ. Methods :60 cases of facial superficial degree Ⅱ burn patients and 60 cases of facial deep degree Ⅱ burn patients were enrolled in this study and were observed by randomized, double-blinded and placebo-controlled protocol. An area of degree Ⅱ burn wounds from each patient was divided into control (C) and treatment (T) portions. The wound in C was treated with normal saline gauzes while that in T with rhEGF gauzes. Dressing change everyday until the wounds healing. Results :In therapeutic group, the wounds healing speed is accelerated, the healing time is shorten (P 〈 0.01 ) and the healing ratio on synchronization is promoted (P 〈 0. 01) than the control group. The pain without exacerbating and with less scars formation. Without obvious side effects or dys-response in the therapeutic process. Conclusions:rhEGF can accelerate obviously the healing speed,lessen the healing time on the facial burn wounds of degree Ⅱ and can promote the healing quality with less scars formation, rhEGF is effective and safe to the wounds.
