摘要 目的:探讨PTEN、EGFR在人胸腺瘤中的表达意义及二者间的相关性。方法:采用免疫组织化学SP法,检测20例良性胸腺瘤、18例恶性胸腺瘤和7例胸腺癌中PTEN和EGFR的表达。结果:在3组胸腺肿瘤中PTEN阳性表达率分别为80.00%(16/20)、44.44%(8/18)、28.57%(2/7)(P<0.05),EGFR阳性表达率分别为35.00%(7/20)、72.22%(13/18)、85.71%(6/7)(P<0.05)。PTEN和EGFR的阳性表达率与胸腺瘤的肿瘤性质、Masaoka分期及淋巴结转移等参数有关(P<0.05),与是否合并重症肌无力(MG)无关。PTEN与EGFR表达呈负相关(r=-0.632,P<0.01)。结论:PTEN的失表达和EGFR的过表达在胸腺瘤的发生、浸润和转移中起着重要作用,PTEN和EGFR的异常表达密切相关,二者的联合检测有助于胸腺瘤的早期诊断和判断其恶性程度、侵袭和转移能力。 Objective: To investigate the expression and clinical significance of PTEN and EGFR in human thymoma and their correlations. Methods: The expression of PTEN and EGFR were detected and compared using SP immunohistochemical technique among benign thymomas group (20 cases) ,invasive thymomas group (18 cases) and thymic carcinomas group (7 cases). Results. The positive ratios of PTEN expression was 80.00% (16/20), 44.44%(8/18) and 28.57% (2/7) respectively in the three group(P〈0.05) ;and the positive ratios of EGFR expression was 35.00% (7/20),72.22% (13/18) and 85.71% (6/7) respectively in the three group (P〈0.05). They showed obvious relations with the Levine classification, Masaoka staging and lymph node metastasis (P〈0.05), but not with myasthenia gravis(P〉0. 05). PTEN positive eases were reversely correlated to EGFR in human thymoma(r=-0. 632, P〈0.01). Conclusion: Down-regulating or absence of PTEN expression and increased EGFR expression might play an important role in the tumor origin,invasiveness and metastasis of thymoma. The expression of PTEN was strongly associated with the expression of abnormal EGFR. Assaying of both proteins' expressions simultaneously is helpful for making an early diagnosis of the tumors and judging whether it is malignant or not,and its invasiveness and metastasis,as well as the prognosis.
