颈椎椎弓根螺钉徒手置钉技术的临床可行性研究 被引量:2

摘要 目的:探讨颈椎椎弓根螺钉徒手置钉技术的临床可行性和安全性。方法:采用Axis内固定系统对20例颈椎骨折脱位病例进行经颈椎椎弓根内固定的手术治疗并对手术操作技巧及术中并发症和处理方法等进行分析和总结。结果:共置入螺钉92枚,其中准确置入有81枚,准确率为88%;有11枚螺钉穿破椎弓根皮质,其中1枚疑有血管损伤,另有1枚术后出现神经刺激症状,余未发现有脊髓、神经及血管等损伤;有3枚螺钉在初次置钉后有松动感,虽经有效处理,但其中l枚在术后3个月出现退钉现象。结论:颈椎椎弓根螺钉徒手置钉技术是安全可行的。熟悉颈椎解剖和手术操作技巧以及置钉个体化等是决定手术成功与否的重要因素。 Objective: To probe the feasibility and safety of bare-handed fixation of cervical pedicle screws. Method: Twenty cases suffered from cervical fracture and dislocation were operated with Axis system for internal fixation. Operative skills and complications were analysed and related treatment scheme delivered. Result: Eigty-one of total Ninety-two screws(88%) were settled successfully. Eleven screws penetrated the wall of pedicle. Blood vessel injury was found in one case. Nerve injury was found in another case. Three screws loosed and one of them withdrew three months after operation although effective treatment delivered. Conclusion: This technique was identified to be safe and feasible. The necessary factors for a successful operation contains familiarity of the cervical anatomy, skill of manipulation and individuality of screw fixation.
