摘要
目的探讨噬血细胞综合征的骨髓像特点及检验结果分析。方法回顾分析26例噬血细胞综合征的骨髓像特点及检验结果。结果16例(61.5%)与感染相关;以病毒感染最多见。骨髓像特点:HSP早期骨髓增生活或增生减低,粒红比减低,重症HPS骨髓增生重度减低,粒、红、巨三系减少,全部骨髓片找到吞噬了完整形态、数量不等的有核细胞、红细胞或血小板的噬血细胞。90%以上患者外周血出现白细胞和血小板二系或三系减低。90%以上患者TG高、SF高、Fib低和APTT延长。结论骨髓像找到吞噬了完整形态、数量不等的有核细胞、红细胞或血小板的噬血细胞为HPS诊断依据,HPS患者出现TG升高、SF升高,FIB减低和APTT时间延长,临床上有上述情况,必须警惕HPS发生。
Objective To approach into the characteristics of myelogram and blood test analysis of hemophagocytic syndrome. Methods The characteristics of myelogram and blood test results in 26 patients with hemophagocytic syndrome were respectively reviewed. Results 16 cases（61.5%） were related to infection, specially infected by viruses. The characteristics of myelogram showed： The early stage ratio of hemophagocytic syndrome was hyperproliferation or hypoplasia, with the decreasing of granulocytes to erythrocytes. The marrow cells of severe hemophagocytic syndrome was popular hypoplasia, with the decreasing of granulocyte system, erythrocyte system and magalokarycyte system. All above cases were found hymophagocytes phagocytized karyocytes with normal morphology, erythrocytes or platelets. The series of granulocyte and （or） platelet in peripheral blood were decreased over 90% of eases. The increasing of triglyceride, serum ferritin, activated partial thromboplastin time and decreasing of fibrinogen were over 90% of cases. Conclusions Hemophagocyte phagocylized karyocytes with normal morphology, erythrocytes or platelets is the diagnostic criteria of hemophagocytic syndrome. Most of cases appeared the increasing of triglyceride, serum ferrtin, activated partial thromboplastin time and decreasing of fibrinogen.
