摘要
目的:探讨急性白血病患儿survivin、bcl-2、mdr1 mRNA表达及临床意义。方法:用RT-PCR的方法检测患儿骨髓survivin、bcl-2和mdr1的mRNA。结果:初诊患儿survivin、bcl-2和mdr1的阳性表达率高于对照组。初诊急性淋巴细胞性白血病和急性髓细胞性白血病survivin、bcl-2、mdr1阳性表达率无显著性差异。完全缓解期患儿survivin和bcl-2阳性表达明显低于初诊白血病患儿。复发组survivin、bcl-2、mdr1三者阳性率明显高于缓解组。survivin阳性和阴性表达者外周血白细胞总数无显著性差异(P>0.05)。结论:survivin和bcl-2在急性白血病患儿表达增高,其表达与急性白血病的类型和外周血白细胞总数无关。survivin、bcl-2和mdr1在儿童白血病的共表达可能提示预后不良。
Objective: To explore the expression of survivin, bcl-2 and mdrl in acute leukemia in children and its clinical significance. Methods: The mRNA of survivin, bcl-2 and mdrl in all patients were detected by RT-PCR. Results: The positive expression rates of survivin, bcl-2 and mdrl in the newly diagnosed group were 60.47%(26/43N), 60.47%(26/43N) and 34.88% (15/43N), respectively, all of which were higher than those in the control group (P 〈 0.05). There was no significant difference in the positive expression of survivin, bcl-2 and mdrl between acute lymphocytic leukemia (ALL) and acute myeloid leukemia (AML) group (P 〉 0.05). The positive expression rates of survivin and bcl-2 in the CR group were significant lower than those in the newly diagnosed group (P 〈 0.05). There were 3 patients and 2 patients with positive co-expression of survivin, bcl-2 and mdrl in newly diagnosed group and RE group respectively. The WBC was not significantly different between positive survivin and negative survivin group (P 〉 0.05). Conclusions: survivin, bcl-2 and mdrl have higher expression in children with AL, which may contribute to the pathogenesis and the progress of the leukemia. The expression of survivin and bcl-2 is not related to the type of leukemia and the WBC. The co-expression of survivin bcl-2 and mdrl may indicate the poor prognosis of leukemia in children.
