神经外科病人术后早期营养支持研究

摘要 目的探讨神经外科病人术后早期营养支持。方法将90例术后神经外科病人随机分为A、B、C三组,每组30例。A组为肠外营养、自制匀浆营养液———康能液序贯营养支持;B组全肠外营养;C组对照组常规输液治疗。观察术后9天营养和免疫指标变化情况。结果A组和B组术后体质量、前清蛋白、清蛋白均高于对照组(P<0.05),而A组与B组间无差异显著性意义。A组、B组术后CD4+、CD4+/CD8+均明显高于C组(P<0.01),且A组术后CD4+、CD4+/CD8+显著高于B组(P<0.05)。结论神经外科病人术后早期肠外营养加自制匀浆营养液序贯营养支持可改善术后病人近期营养和免疫状况。 Objective To study the early postoperative nutrition support in patients with of neurosurgery. Methods Ninety patients were randomly divided into A( n = 30), B ( n = 30) and C ( n = 30) groups. Patients in A and B groups were given nutrition support from the first postoperative day. Nutritional and immune parameters were detected on the day before operation and postoperative nineth day. Results The postoperative levels of BW,PA,ALB in A and B groups were significantly higher than that in control group( P 〈 0.05) ,and there were no statistical differences between A and B groups. The postoperative percentange of CD4^ + 、CD4^+/CD8^+ in A or B group was significantly higher than that in control group ( P 〈 0.01 ). The prstoperative levels of CD4^+ and CD4 ^+/CD8^ + in A group were higher than that in B group( P 〈 0.05). Conclusion Eearhy postoperatire nutritional support can improve nutritional and immune status of postoperative cerebral disease patient which is berrer than parenteral nutritional in cellular immunity and restoring of intestinal function.
