摘要 目的研究氢氯噻嗪(hydrochlorothiazide,Hyd)对大鼠胰岛β细胞功能影响的机制。方法采用放射免疫分析法和电子显微镜技术,系统研究Hyd对大鼠胰岛β细胞分泌功能、血清细胞因子以及胰岛β细胞超微结构的影响。结果Hyd 10.5mg.kg-1.d-1,灌胃给药,连续用药21d,可使大鼠血糖含量明显降低而血清胰岛素(INS)水平明显升高(P<0.05),血清白细胞介素-6(IL-6)水平、肿瘤坏死因子-α(TNFα)水平均明显升高(P<0.01或0.05)。Hyd 21.0mg.kg-1.d-1组IL-6,TNFα明显升高(P<0.01或0.05),大鼠血糖含量明显升高(P<0.05)、血清INS水平明显降低(P<0.05);电镜观察胰岛β细胞超微结构发生典型细胞退行性改变。结论Hyd低剂量促进胰岛β细胞分泌功能而高剂量抑制胰岛β细胞分泌功能;长期应用Hyd可引起大鼠血清IL-6,TNFα含量明显升高,且使胰岛β细胞超微结构出现退行性改变。 Objective To explore the function changes of pancreatic heta cells of rats and its mechanism which were induced by longtime-use of hydrochlorothiazide( Hyd). Methods The serum insulin, IL-6 and TNFα was measured by radioimmunoassay (RIA) and the ultra-structure of pancreatic beta cells was observed with electron mieroscope. Results The level of serum insulin of 10.5mg·kg^-1·d^-1. Hyd group was obviously higher than that of the control group( P 〈 0.05 ) ; while the level of BS obviously lower( P 〈 0.05 ) ; the level of serum insulin of 21.0mg·kg^-1·d^-1 Hyd was obviously higher( P 〈 0.05), and the level of BS obviously lower(P 〈 0.05). The levels of IL-6 and the serum TNFα levels in Hyd groups were signifieantly increased when compared with that of the control group( P 〈 0.01 or 0.05 ) ; Under the electron microscope the ultra-micro structure of pancreatic beta cells appeared the classical apoptosis changes. Conclusion Hyd in low dosage group may promote the function of panereatic beta cells, while regress it in high dosage group; Hyd may cause the levels of serum IL-6 and TNFα go up obviously;The longtime-ased big dosage of Hyd may insult in apoptosis in pancreatic beta cells.
