摘要
目的 评价高频振荡通气(HFOV)治疗西宁地区新生儿肺透明膜病(NRDS)的疗效。方法 对12例NRDS采用HFOV治疗,采用高容量策略。频率10~15HZ,通过调整平均气道压、振荡压力幅度、吸入氧浓度等参数,以达到吸入氧浓度≤0.4,经皮氧饱和度≥0.9,使PaCO,维持在35~45mmHg。结果 治愈9例,死亡1例,自动出院2例,9例患儿行HFOV治疗满24h,治疗平均气道压由(12±3.5)cm H2O降至(8.0±3.2)cm H2O(P〈0.05)。吸入氧浓度由(0.80±0.10)降至(0.35±0.05)(P〈0.05)。治疗过程中心率、血压无明显改变。对7例追踪头颅CT,除1例侧脑室扩大外,余均无改变。结论 提示HFOV用于治疗NRDS是一种安全有效的机械通气方法。
Objective To study the effect of high - frequency oscillatory ventilation newborn with neonatal respiratory distress syndrome in Xining district. Methods Twelve infants were treated with high - frequency oscillatory ventilation, Rate 10 -15 HZ, in order to make fractional inspired oxygen ≤0.4, Saturation oxygen ≥0.9, 35 〈 partial pressure of carbon dioxide 〈45mmHg via adjust average pressure and oscillatory pressure and inspired oxygen ratio, etc. Results Nine cases were cured, one was dead, two cases left hospital deciding by themselves. Average pressure was reduced from ( 12 ± 3.5 ) cmH2O to ( 8.0 ± 3.2 ) cmH2O ( P 〈 0.05 ), fractional inspired oxygen from (0.8 ± 0.1 ) to (0.35 ± 0.05 ) ( P 〈 0.05 ) during first 24 - hour treatment for nine cases. There were no heart rate and blood pressure changes. Seven cases were follow - up; CT shows no lateral ventricle enlarged except one, the rest all - normal. Conclusion high - frequency oscillatory ventilation is an encouraging ventilation method for newborn with neonatal respiratory distress syndrome.
