子宫内膜癌组织中COX-2和ER及PR的表达及临床意义 被引量:4

摘要 目的:探讨环氧化酶-2(cyclooxygenase-2,COX-2)、ER及PR在子宫内膜癌(endometrialcarcinoma,EMC)组织中的表达、相互关系及其临床意义。方法:应用免疫组化技术对68例EMC组织中COX-2、ER及PR进行检测,分析其与临床病理学参数的关系,并选取20例正常子宫内膜作为对照。结果:COX-2、ER和PR在68例EMC组织中的阳性表达率分别为67·6%(46/68)、55·9%(38/68)和52·9%(36/68),而正常的子宫内膜未见COX-2的表达。COX-2阳性表达与EMC的组织病理类型和组织学分级均有明显相关,χ2值分别为4·095和6·582,P值分别为0·043和0·009;而与年龄、手术病理分期、肌层浸润深度和淋巴结转移无关,χ2值分别为1·789、0·718、0·729和0·360,P值分别为0·181、0·397、0·393和0·549。ER和PR的阳性表达与组织病理类型、组织学分级及淋巴结转移密切相关,χ2值分别为6·123、7·740和4·424,P值分别为0·013、0·005和0·035;χ2值分别为4·494、5·259和4·566,P值分别为0·034、0·022和0·033;而与年龄、手术病理分期和肌层浸润深度无关,χ2值分别为0·06、0·793和0·194,P值分别为0·939、0·373和0·660;χ2值分别为1·249、1·776和0·915,P值分别为0·264、0·183和0·339。结论:COX-2在EMC的发生、发展中起重要作用,对选择性COX-2抑制剂预防治疗EMC起指导作用,ER、PR的缺失表达与EMC的恶性程度密切相关,对分析EMC的预后和指导临床内分泌治疗均有重要意义。 OBJECTIVE: To study the expressions and clinical significance of COX-2,ER and PR in endometrial carcinoma. METHODS: Sixty-eight cases of endometrial carcinoma were examined by the method for COX-2,ER and PR.RESULTS: The positive rates of COX-2, ER and PR were 67.6%, 55.9% and 52.9% respectively. COX-2 expression was correlated with both histologic type and histologic differentiation,x^2=4. 095,6, 582,P=0.043, 0.009. However,the relative contents of COX-2 did not correlate significantly with Age, Pathologic stage, myometrium depth of invasion and Lymph node metastasis, x^2 = 1. 789, 0. 718, 0. 729,0. 360,P=0. 181,0. 397,0. 393 and 0. 549. Both ER and PR were correlated with histologic type, differentiation and lymph node metastasis, x^2=6. 123, 7. 740,4. 424, P=0. 013, 0.005 and 0.035;x^2 =4.494, 5.259 and 4.566, P=0.034, 0.022 and 0.033; tdonoever, ER and PR expressions were not significantly correlated with age, pathologic stage, myometfium depth of invasion, x^2 = 0. 06, 0. 793 and 0. 194, P = 0. 939, 0. 373 and 0. 660; x^2 = 1. 249, 1. 776 and 0. 915, P = 0. 264, 0. 183 and 0. 339. CONCLUSION:COX-2 may play an important role in the development of endometfial carcinoma. It is important in directing the prevention and treatment of endometrial carcinoma by cyclooxygenase inhibitors. Both ER and PR are important in predicting the prognosis and directing the treatment of endometrial carcinoma.
