摘要
采用多平面经食道超声心动图对16例房间隔缺损患者(男3例,女13例,平均年龄31.5岁)的分流量进行了定量研究,均由手术和心导管检查所证实。分流量=VIT·A·HR·10-6(VTI为跨房间隔缺损的流速积分,A为缺损面积,HR为心率)。结果显示多平面经食道超声心动图测量的房间隔缺损分流量与心导管测量的Qp/Qs高度相关(r=0.94,P<0.001),经胸超声心动图测量的Qp/Qs与心导管测量的Qp/Qs仅中度相关(r=0.76,P<0.001),且多平面经食道超声心动图测量的房间隔缺损的最大直径及面积亦与心导管测量的Qp/Qs高度相关(r分别为0.92、0.94,P<0.001),表明多平面经食道超声心动图为定量评价房间隔缺损分流量提供了一种新的方法。
Transesophageal Dappler color flow imaging was performed in 16 patients (13 women and 3 men age 14~47 years, mean age 31. 5 years ) with atrial septal defects demonstrated by both cardiac catheterization and sugery. The shunt volume was calculated as a product of the defect area,velocity time integral and heart rate (Q=A·VTI·HR·106). The results showed that there was medial correlation of the pulmonary to systemic blood flow ratio measured by transthoracic echocardiography and cardiac catheterization (r=0. 76, P<0. 001 ). The shunt flow volume and area of atrial septal defect measured by transesophageal echocardiography showed a good correlation with the pulmonary to systemic blood flow ratio obtained at cardiac catheterization (r=0. 94, P<0. 001 ). It is calculated that multiplane transesophageal echocardiography is a new method for the assessment of the shunt volume in atrial septal defect.
