糖尿病新生血管性青光眼视神经血管内皮细胞在透射电镜下的改变 被引量:1

摘要 目的通过对糖尿病新生血管性青光眼患者视神经的血管内皮细胞透射电镜下形态改变的观察,分析其超微结构改变的临床意义。方法6例因糖尿病新生血管性青光眼行眼球摘除术患者的眼球,术中取球后视神经2mm,经戊二醛固定后,在透射电镜下观察视神经血管内皮细胞超微结构改变。结果糖尿病新生血管性青光眼患者的视神经血管内皮细胞表现出显著的超微结构改变,主要表现血管内皮肿胀明显、核肿胀、细胞绒毛减少、内皮细胞坏死缺失、血管腔狭窄闭塞、血栓形成。结论糖尿病新生血管性青光眼患者视神经血管内皮细胞出现显著超微结构改变,并与血栓形成互为因果,可能是迅速加重视功能损害的主要病理基础之一。 Objective Diabetic neovascular glaucoma is a serious complication of the diabetes mellitus when rubeosis irdis occurs and is difficult to treat. This study was curried out to observe the ultrastructural changes of vascular endothelium cells in optic nerve with diabetic neovascular glaucoma under transmission electron microscope (TEM) and analyze the meaning of the changes. Methods Six cases with diabetic neovescular glaucoma were treated with ophthalmectomy from January 2000 to June 2004 in Nanjing General Hospital of Nanjing Command PLA. The optic nerve was excised 2 mm post eyeball in these patients and fixed by the glutaral for the examination of TEM (JEM-1200EX). Results The ultrastructure of vascular endothelium in optic nerve was altered by diabetic neovascular glaucoma, including the decrease of villi of the endothelial cell in capillary vessel and cell apoptosis and cleavage. Some endothelial cell necrosis and loss, vascular cavity stricture, venulae endothelium swelling, vacuole degeneration, elastic fibers swelling and twisting could be seen in cytoplasm, and the vascular cavity was very stricture in the specimen. Some new thrombosis of vascular cavity was connected with the basal membrane of vessel. Conclusion The ultrastructural changes of the vascular endothelium cells might be one of the pathogenesis bases of the irreversibility optic function damage in the diabetic neovescular glaucoma.
