氯吡格雷对缺血性心肌病患者心功能的影响 被引量:3

The effect of clopidogrel on cardiac function in patients with ischemic cardiomyopathy.

摘要 目的探讨氯吡格雷对缺血性心肌病(ischemiccardiomyopathy,ICM)患者心功能的影响。方法缺血性心肌病伴心力衰竭患者40例,分为氯吡格雷组和对照组各20例,对照组应用现代心力衰竭治疗模式治疗,氯吡格雷组应用现代心力衰竭治疗模式加用口服氯吡格雷(75mg/d)治疗。采用UCG评价两组治疗前及治疗4周后的左心室射血分数(leftventricularejectionfraction,LVEF),并比较两组心功能分级的改变。结果治疗后,对照组LVEF由0.42±0.08增加至0.49±0.09,氯吡格雷组则由0.42±0.07增加至0.58±0.09,两组治疗后的LVEF比较差异有统计学意义(P<0.05);两组治疗前心功能分级比较差异无统计学意义,治疗后氯吡格雷组的心功能分级(Ⅰ级13例,Ⅱ级7例)比对照组(Ⅰ级6例,Ⅱ级14例)明显改善,差异有统计学意义(均为P<0.05)。结论氯吡格雷可以改善ICM患者的心功能。 Objective: To investigate the influence effect of clopidogrel on cardiac function in patients with ischemic cardiomyopathy (ICM). Methods: Forty cases of ICM with heart failure (HF) were allocated into control group and clopidogrel group. Each group had 20 cases. The control group patients were treated with modarn heart failure guideline. The clopidogrel group patients were given oral clopidogrel (75 mg/d) in addition to the standard therapy. The left ventricular ejection fraction (LVEF) were evaluated by ultrasound cardiogram (UCG) before and 4 weeks after treatment. The change of cardiac function grades was compared between two groups at the same time. Results: After treatment, the LVEF increased from 0. 42 ±0.08 to 0. 49 ± 0. 09 in control group, and from 0. 42±0. 07 to 0.58 ± 0. 09 in clopidogrel group, respectively. The LVEF difference between two groups had statistic significance ( P 〈 0. 05 ). Although the eardiac function grades had no difference before treatment, the cardiac function grade improved more significantly in elopidogrel group ( Ⅰ grade 13 cases, Ⅱ grade 7 cases) than that in control group (Ⅰ grade 6 cases, Ⅱgrade 14 cases) (P 〈0. 05). Conclusion: Clopidogrel can improve cardiac function in ICM patients.
