摘要 目的评价CT三维成像诊断骨关节疾患的临床价值,提高临床诊疗水平。方法回顾性分析357例患者的三维影像资料,全部病例经手术或临床确诊。重点观察三维重建图像的表现及诊断准确率,与常规CT诊断准确率比较并进行χ2检验处理。重建方法包括表面遮盖法(SSD)、多层面重建法(MPR)、最大强度投影法(MIP)和容积重建法(VR)。结果三维成像诊断创伤性病变257例,非创伤性病变100例,诊断准确率为98.3%。统计学处理显示CT三维成像诊断准确率高于常规CT(P<0.05)。三维成像能清楚、立体显示骨折线的长短、形态、走向,碎骨片及肿瘤或肿瘤样病变的大小、形态及周围结构。结论CT三维成像显示骨关节结构及病变直观、清楚,特别是复杂结构或深部关节。其诊断准确率高,同时有利于临床治疗方案的选择。 Objective To evaluate the clinical value of CT three- dimensional(3 - D) imaging on diagnosing of hone and joint diseases. Methods The imaging findings of 357 cases proved by operation or clinic were analyzed retrospectively by the 3 - D imaging. The diagnosis accuracy of 3 - D imaging was compared with that of routine CT. 3- D images were obtained by surface shade display(SSD), multi - planar reconstruction(MPR), maximum intensity project (MIP) and volume rendering (VR). Results In 357 patients, 257 cases were traumatic disease, 100 cases were non- traumatic disease. The diagnosis acctLracy of 3 - D imaging was 98.3 %, which was higher than that of routine CT (P〈 0.05). 3 - D imaging could show the length and shape of fracture line, and the size, shape and location of tumorous focus and fragment of bone. Conclusion CT 3 - D imaging can show the structure and focus of hone and joint directly and dearly, which has the outstanding advantage to showing complicated structures or deep joint. The diagnosis accuracy of 3 - D imaging is high, and helpful in the choice of clinical treatment.
