脑电生物反馈对诊断和治疗儿童注意缺陷多动障碍的临床探讨 被引量:7

摘要 目的:探讨脑电生物反馈对儿童注意缺陷多动障碍(ADHD)的诊断价值和治疗效果。方法:对121例符合美国精神病学会出版的《精神病诊断统计手册》第4版DSM-Ⅳ诊断标准的ADHD患儿用脑电生物反馈诊断治疗仪进行诊断和训练治疗,增强16~21Hz的β波,抑制4~8Hz的θ,观察θ/β比值的变化;并选用有关神经心理测试量表对其ADHD表现的症状行为进行测评,主要方法为Achenbach儿童行为量表(CBL)、Conners教师评定量表(TRS)及美国简化康奈尔儿童行为量表(简称康氏表)评定。结果:121例ADHD儿童脑电θ/β比值的均值为4.5,与常用的指标5.0相比有所降低。所有患儿经1~4个疗程后作自身治疗前后比较,各疗程与治疗前相比脑电θ/β值均显著下降(P〈0.01),治疗2个疗程以上,CBL、TRS及康氏表评定分值有显著降低(P〈0.05);治疗4个疗程后,三种量表评分与治疗1个疗程相比也有明显下降(P〈0.05)。经脑电生物反馈治疗后ADHD患儿的注意力、认知功能有所提高,多动症状明显减少。结论:脑电生物反馈仪所测得的θ/β值对ADHD患儿有辅助诊断价值,在临床应用中可适当降低此值,以减少漏诊;经治疗后注意力、行为问题均有不同程度的改善,治疗效果显著,以规范治疗4个疗程为宜。 Objective : To study the effect of EEG biofeedback diagnosis and treatment to attention deficit and hyperactivity disorder (ADHD) in children. Methods: 121 children diagnosed as ADHD according to criteria of DSM-Ⅳ were received electroencephalographic (EEG) biofeedback diagnosis and treatment by A620 instrument using a bipolar placement. The EEG Biofeedback protocol reinforced the enhancement of β brain waves (16-21 Hz) and the suppression of θ brain waves (4- 8 Hz). The changes were observed before biofeedback and after each course. Psychological evaluation included combined Achenbach index of child behavior checklist (CBCL), Conners index teacher rating scale (TRS) and simplified Conners Behavior Rating Scales. Results: The average θ/β values of 121 ADHD children were 4.5, which was lower than the common criteria. The θ/β values of brain waves were significant decline among pre-treatment and post-treatment of each course in 121 children by treatment of EEG biofeedback (P 〈0.01 ). The indices of psychological evaluations (CBCL, TRS and Conners) were descending after treatment of 2 courses ( P 〈 0.05 ). There were also obvious lessening in the three indices between the treatment of 4 courses and 1 course (P 〈 0.05 ). Children received EEG biofeedback treatment were enhanced in attention and cognitive ability, and lessened in the symptoms of hyperactivity disorder. Conclusion: The results indicated that the EEG biofeedback combined with criteria of DSM-Ⅳ was a valuable way for ADHD diagnosis in children. In clinic diagnosis, the θ/β values were suggested to reduce slightly avoiding missed ADHD children. The ADHD children's attention and behavior problems could be improved through EEG biofeedback treatment. It was suited for 4 courses treatmeht according to the results of our study.
