摘要
目的观察野战环境战地两种低张胶体液对腹腔海水浸泡伤实验犬的救治作用。方法杂种犬20只,随机均分为对照组(A组)、生理盐水救治组(B组)及低张胶体(右旋糖酐70)救治组(C组)及低张胶体(右旋糖酐40)救治组(D组),观察每一组腹腔海水浸泡后的存活时间及血渗透压、乳酸水平变化。结果C、D两组平均存活时间分别为5.8和6.3h,显著长于A(2.4h)、B(4.1h)两组(P<0.01);腹腔海水浸泡后1hC、D两组血浆渗透压分别为(366.1±8.4)mmol/L(、365.4±7.7)mmol/L,乳酸分别为(8.9±1.8)mmol/L、(8.7±1.7)mmol/L,均显著低于A组分别为(374.3±11.2)mmol/L(、12.4±3.1)mmol/L(P<0.05)。腹腔海水浸泡后3hC、D组血乳酸水平分别为(9.1±1.7)mmol/L(、9.4±2.3)mmol/L,均显著低于B组(11.4±1.9)mmol/L(P<0.05)。结论腹腔海水浸泡伤战地早期补入低张胶体液有助于缓解机体高渗状态,改善机体代谢,延长创伤动物生命,两种低张胶体液具有类似作用。
[Objective] To observe early effect of two kinds of hypotonic liquid containing colloid on celiac seawater immersion wound in field aid. [Methods] Twenty dogs were randomly divided into control group(group A), saline aid group (group B) and compositive aid group (group C). Survival time was observed and serum levels of osmotic pressure and lactic acid were measured after celiac seawater immersion in every groups. [Results] Average survival tin,e was significantly longer in group C and D (5.8 h and 6.3 h, respectively) than that in group A (2.4 h) and B (4.1 h) (P 〈0.01). Serum levels of osmotic pressure (366.1±8.4) mmol/L and (365.4±7.7) mmol/L, respectively and lactic acid (8.9±1.8) mmol/L and (8.7±1.7) mmol/L, respectively) were decreased in group C and D 1 h after celiac sweater immersion (P 〈0.05), compared with group A (374.3±11.2) mmol/L and (12.4±3.1) mmol/L, respectively. Levels of lactic acid in group C and D (9.1±1.7) mmol/L and (9.4±2.3) mmol/L, respectively) were lower than those in group B (11.4±1.9) mmol/L 3 h after celiac sweater immersion (P 〈0.05). [Conclusion] Early infusion of hypotonic liquid containing colloid and celiac debridment can reduce hypertonic conditions, improve metabolism and prolong survival time in field aid of celiac seawater immersion wound. Hypotonic liquid containing colloid (Dextran 70) and hypotonic liquid containing colloid (Dextran 40) can exert similar satisfactory therapeutic effects in field aid.
