摘要
目的评价通过植片和植床大小匹配、间断缝线、选择性拆线相结合，对穿透性角膜移植术后屈光控制的效果。方法17例（20眼）根据术前不同屈光状态采用不同角膜移植片、植床大小及间断缝线技术进行穿透性角膜移植。术后1个月、3个月、6个月、12个月进行视力、角膜曲率、角膜地形图及屈光检查并行选择性拆线，通过比较视力、角膜屈折力、散光、等效球镜，评价术后不同时间的视光学状态。结果裸眼视力〉0．5者11眼，0．3～0．5者5眼，0．1～0．3者4眼；平均裸眼视力0．58±0．37，最佳矫正视力0．96±0．27。首次拆线前角膜屈折力（37．26±2．85）D，拆线后最终角膜屈折力（41．52±5．15）D，2者有统计学差异，P〈0．05。拆线前角膜散光平均（4．30±4．60）D，拆线后1个月、3个月、6个月分别为（3．29±3．81）D、（2．07±3．29）D、（2．13±3．21）D，最终散光（1．65±2．73）D。术后等效球镜1个月、3个月、6个月、12个月分别是（7．20±2．63）D、（5．23±3．49）D、（4．01±5．26）D、（1．07±3．21）D。术后1个月与3个月、6个月、12个月对照差异显著，术后3个月、12个月之间有明显差异，P〈0．05，术后3个月、6个月之间无明显差异，P〉0．05。结论植片植床大小匹配、间断缝线和选择性拆线相结合可以有效改善PKP术后的屈光状态，提高视力，达到控制屈光的目的。[眼科新进展2006；26（3）：206—207，209]
Objective To evaluate the effects of the penetrating keratoplasty （PKP） with methods combined different sizes of grafts from both donor and recipient cornea, interrupted suture and selective suture detached on the penetrating reratoplasty refraction control. Methocls Twenty eyes of 17 patients who underwent PKP were studied. The visual acuity, corneal curvature, spherical equivalent were measured before operation and at 1 month,3 months, 6 months, 12 months after operation intervals. To detach the interrupted suture selectively to decreases the postoperative astigmatism. Results The postoperative visual acuity was greatly improved, the mean uncorrected visual acuity （UCVA） was 0.58 ± 0.37, the mean best correct visual acuity （BCVA） was 0.96 ± 0.27, the suture was removed initially at （6.28 ± 3.21 ） months （ranges from 1.5 months to 12.5 months）, the mean corneal curvature was （37.26 ± 2.85） D before the first suture removal and changed to （ 1.52 ± 5.15） D after all suture removal. The spherical equivalent were （7.20±2.63） D,and （5.23 ±3.49） D,（4.01 ±5.26） D, （1.07 ± 3.21 ） D, respectively 1 month; 3 months; 6 months; 12 months after operation, the mean corneal astigmatism was （4.30 ± 4.60） D before suture removal,it were （3.29±3.81） D,（2.07±3.29） D,（2.13± 3.21 ） D after suture removal 1 month, 3 months, 6 months,and the last astigmatism was （ 1.65 ± 2.73） D. Conclusion The combined methods can obviously reduce the postoperative refractive errors, improve visual acuity and control the postoperative visual performance in deed.
