摘要
目的对巨块型肝细胞癌边缘部的螺旋CT征象和超声征象进行对照研究,并探讨其病理学基础。方法对手术病理证实或临床确诊的61例原发性巨块型肝细胞癌的边缘部的螺旋CT和超声征象进行对照分析。结果①61例病灶中,CT显示病灶边界清晰的有7例,其中4例超声显示病灶边界清晰,3例超声显示病灶边界大部分清晰。CT显示病灶边界大部分清晰的有20例,此20例超声亦显示为病灶边界大部分清晰。CT显示病灶边界部分清晰的有7例,其中5例超声显示病灶边界部分清晰,另外2例超声显示病灶边界清晰。②61例病灶中,CT显示肿瘤包膜不完整的有13例,其中8例超声显示为晕征不完整,5例超声无晕征显示。CT显示肿瘤有部分包膜的有20例,其中超声显示晕征完整的有1例,晕征不完整的有8例,部分晕征的有8例,无晕征显示的有3例。CT显示无包膜的有28例,27例超声无晕征显示,仅1例超声有部分晕征。③17个病理点位于病灶边缘部,CT显示有包膜、超声显示有晕征的一共有7个病理点,此7个病理点在镜下均见病灶与正常肝实质之间有纤维分隔。结论①CT和超声对于巨块型肝细胞癌边界的显示相当,但CT对于病灶后缘的显示优于超声。②CT显示病灶边缘的包膜和超声显示病灶边缘的晕征一致性较好,巨块型肝细胞癌周围的晕征主要是由病灶与邻近肝实质之间的纤维分隔构成。
Objective To study the correlation between helical CT and ultrasound feature of the border of huge hepatocelllular carcinoma(hHCC). Methods Helical CT and ultrasound features of the borders of the tumor in 61 cases were analyzed, and all cases were confirmed as hHCC by histopathology or clinic in our hospital. Results ① In 61 cases, CT displayed clear borders in 7 ones, among them ultrasound displayed clear border in 4 cases, almost clear borders in 3 ones;CT displayed almost clear border in 20 ones, ultrasound also displayed almost clear borders;CT displayed partial clear borders in 7 ones, among them ultrasound displayed partial clear borders in 5 cases, clear borders in ones; CT displayed unclear borders in 27 ones, among them ultrasound displayed unclear borders in 25 cases, partly clear borders in 2 ones. ②In 61 cases, CT displayed almost intact tumor pseudo -capsule in 13 ones, among them ultrasound displayed almost intact halo ring in 8 cases, no halo ring in 5 ones;CT displayed partial tumor pseudo -capsule in 20 ones, among them ultrasound displayed intact halo ring in one case, almost intact halo ring in 8 ones, partial halo ring in 8 ones, no halo ring in 3 ones; CT displayed no tumor pseudo - capsule in 28 ones, among them ultrasouhd displayed partial halo ring in one case, no halo ring in 27 ones. ③ 17 histopathologic specimens were located in borders of lesions, 7 specimens which CT displayed pseudo - capsule and ultrasound displayed halo ring showed fibrous septum between lesion and hepatic parenchyma under light microscope. Conclusions ① Helical CT and ultrasound displaying the border of the lesion are consistent with each other, however, helical CT is superior to ultrasound in displaying the back border of the lesion. ② Helical CT displaying the tumor pseudo- capsule and ultrasound displaying halo ring are comparatively consistent with each other, halo ring in hHCC is mainly formed by the fibrous septum between rumor and hepatic parenchyma.
