摘要
目的比较超声造影(CEUS)和增强CT显示肝肿瘤的血流灌注特点,探讨对肝良恶性肿瘤的诊断价值。方法对92例肝占位性病变进行CEUS和增强CT检查,比较两种技术在不同时相的增强及灌注特征。结果原发性肝癌、血管瘤CEUS和增强CT的增强类型比较差异无显著性意义(χ2检验,P>0.05),肝转移癌两者比较差异有显著性意义(P<0.05)。其中30例肝恶性肿瘤CEUS显示的病灶数目为134枚,增强CT显示的病灶数目为118枚。结论CEUS和增强CT能敏感地显示肝良恶性肿瘤的血供特征,CEUS对肝转移癌微小血管的灌注情况及发现微小病灶方面优于增强CT,可有效地评价肿瘤血管化程度,为肝癌的早期诊断提供更多信息。
Objeetive To compare the perfusion features of liver tumors with contrast-enhanced ultrasound (CEUS) and contrast-enhanced CT and evaluate their diagnostic value. Methods 92 cases of hepatic space-occupylng lesions were detected with CEUS and contrast-enhanced CT respectively. Enhancement patterns and perfusion characters of liver tumors were compared in different phases. Results Using CEUS and contrast-enhanced CT, there were no significant difference in the enhancement patterns of primary liver carcinoma and hepatic hemangioma (X^2 test, P〉0.05). There was statistical difference in liver metastases (P(0.05). Of the 30 liver malignant tumors, CEUS and contrast-enhanced CT showed 134 and 118 lesions respectively. Conclusion Both CEUS and contrast-enhanced CT could display hemodynamic characters of liver tumors sensitively. CEUS was superior to contrast-enhanced CT in detecting perfusion of micro-vascularity, especially in finding smaller lesions. So CEUS could evaluate the vascularization of liver tumors effectively and provide more information in early diagnosis of liver carcinoma.
