摘要
目的:根据多年成功救治经验,总结采用烧伤皮肤再生疗法救治严重烧伤的治疗要点。方法:提供了作者几家单位2002年至2004年底收治的110例严重烧伤患者的病例总结以及典型病例的介绍,这些患者均采用烧伤皮肤再生疗法进行救治。结果:110例严重烧伤病人治愈106例,死亡4例,均为入院已发生严重休克、感染或同时伴有吸入性损伤病情严重者。休克期渡过平稳者98例。深Ⅱ度及浅Ⅲ度患者愈合无疤痕,皮肤弹性功能正常;小面积深Ⅲ度烧伤创面,部分创面皮缘向中间爬行生长愈合;面积较大的深Ⅲ度烧伤创面行自体皮植皮愈合,愈后留疤痕,但疤痕较平软。结论:本文总结了严重烧伤救治过程中的临床治疗要点,包括了许多临床应用中的实用的诊疗经验,例如急救的“3”通道,烧伤休克补液的具体应用,胃肠道功能的再生保护,创面感染的三个“3”阶段规律和“四大”观测指标,抗生素的应用原则和营养支持的“两到位”原则等等,其目的是提高严重烧伤患者的救治成功率和愈后的生活质量。
Objective： To work out the key points in the application of bum skin regenerative therapy on the basis of the experience in the successful clinical practice in recent years. Method： The authors have successfully applied burn skin regenerative therapy in the treatment of 110 cases of severe bum in different hospitals. The clinical data were analyzed and summarized and typical cases were described. Resuit： 106 out of the 110 severely burned patients were cured and 4 died. Those who died were in a state of severe shock and were complicated by severe infection or inhalation injury when entering the hospital. 98 cases tied over the shock stage smoothly. Patients with deep 2^nd degree and superficial 3^ml degree bum healed without scar formation. The skin had good elasticity and normal function. Small 3rd degree wounds healed by creeping of the skin at the wound edge towards the center. Large 3rd degree wounds were subjected to auto - skin grafting and soft scars formed after healing. Conclusion： The key points in the application of bum skin regenerative therapy emL^dy a lot of valuable practical clinical experience such as the 3 channels in the emergency treatment, the principle of the application of fluid infusion at the shock stage, and the protection of the function of gastrointestinal tract, the principle of the treatment at 3 stages of wound infection and 4 important observation indexes, the principle of the application of antibiotics, the principle of nutrition support, etc. Adhering to these key points will greatly raise the successful rate of the therapy and improve the living quality of the patients.
