摘要 目的:了解重组人生长激素(rhGH)对瓦斯烧伤患者血清胰岛素样生长因子(IGF-1)、白蛋白、血糖以及肿瘤坏死因子-α1(TNF-α1)、白细胞介素-1α(IL-1α)、C-反应蛋白(CRP)等炎性介质的影响,探讨rhGH对烧伤愈合的机制。方法:13例同一批瓦斯烧伤患者随机分为治疗组(7例)、对照组(6例)。治疗组于伤后7天每日早7时接受rhGH皮下注射(0.2u.kg-1.d-1),疗程14天,对照组同时同部位注射等量生理盐水。结果:治疗组用药第7、14天和停药后7天的血清IGF-1、白蛋白含量高于对照组(P<0.01),治疗组用药第7、14天的血清TNF-α、IGF-1、CRP低于同期对照组(P<0.05),治疗组停药后7天的血糖水平低于用药第14天(P<0.01)。结论:rhGH可以提高瓦斯烧伤患者血清IGF-1、白蛋白的水平,降低血TNF-α、IL-1α、CRP的水平,血糖升高的不良反应可随停药而逆转。 Objective To investigate the effect of recombinant human growth hormone (rhGH) on serum inflammy medium such as insulin-like growth fator(IGF-1) ,album,TNF-α,IL-1a,CRP in gas blast induced burn patients of adults,and discuss the mechanism of rHGH on acceleration of burn wound healing. Methods Thirteen burn patients were randomly divided into rHGH group (n =7) and placebo control group (n =6),The rHGH was administered subcutaneously in the dose of 0.2u.kg-~1.d-1 at 7 am every morning in rHGH group for 14 days, while same saline was injected as a placebo in the control group. Results The serum concentration of IGF-1 and aublm in rHGH group (rHGH given from the first and second week,no rHGH for one week) were higher than those in the control group(P〈0,01), that of TNF-α,IGF-1 ,CRP in rHGH group were lower than those in the control group (P〈0.05)and blood suger levels in rHGH group lower to those in the control group (P〈0.01). Conclusion The administration of rhGH in gas blast induced burn patients could improve the serum levels of IGF-1 and album,decrease TNF-α.IL-1aand CRP in the blood of gas blast burn patients, the complication of blood suger increasing was contracted after no rhGH administration.
