螺簧多用弓装置矫治安氏Ⅱ~2错

Treatment of Angle classⅡ division 2 malocclusion by coil-spring utility arch

摘要 目的:探讨采用一种新式矫治装置矫治安氏Ⅱ2错牙合的临床效果。方法:选自第四军医大学口腔医院正畸科门诊安氏Ⅱ2病例20例(其中男9例,女11例),平均年龄12.6岁(10.5~15.8岁)。矫治装置使用多用弓唇弓,在其上套入Ni-Ti推簧,并焊上拉钩,借助Ⅱ类牵引的力值(150 g^200 g),推上颌磨牙向远中移动,并适当牵引下颌后牙向近中移动,最终调整磨牙关系由Ⅱ类成为Ⅰ类。术前术后摄头颅侧位X线片,测量上下磨牙的远中、近中位移。结果:经过平均4.5个月的治疗(3.4~5.8个月),上颌磨牙平均远移2.16±1.3 mm,而下颌磨牙平均向近中移动1.38±0.64 mm。最终磨牙的Ⅱ类咬合关系得到纠正。结论:NiTi螺旋弹簧多用弓装置是一种有效的改正Ⅱ类磨牙关系的治疗方法,其矫治机制是在上颌磨牙远移的基础上,下颌磨牙适当近移共同作用的结果。 Objective:To study the clinical effect of a new method in the treatment of Angle class Ⅱ division 2 malocclusion. Method: 20 subjects (9 boys and 11 girls aged 10.5~15.8 years old) were treated with coil-spring utility arch. Hooks were weld in the arch and classll elastics were used (the force was about 150 ~200 g). The upper molars were moved distally and the lower molars were drifted mesially. The pretreatment and posttreatment cephalometric radiographs were taken in order to measure the distal and mesial movement of the molars. Result: After a mean treatment period of 4.5 months (ranged from 3.4 months to 5.8 months), the class Ⅱ molar relationship was corrected. The distal movement of the upper molars was 2.16 ± 1.3 mm on average, and the mesial movement of the lower molars was 1.38 ± 0.64 mm on average. Conclusion:Coil-spring utility arch is an effective method in the treatment of Angle class Ⅱ division 2 malocclusicn. Its treatment mechanism is the co-operation of the distal movement of the upper molars and the properly mesial movement of the lower molars.
