肝癌及癌旁肝组织胰岛素样生长因子Ⅰ型受体表达的检测

Expression of Insulin-like Growth FactorⅠReceptor and its Clinic Signifcance in Hepatocarcinoma and Pericancinomatous Tissues

摘要 目的检测胰岛素样生长因子Ⅰ型受体(IGF-ⅠR)在肝细胞癌(HCC)、癌旁及正常肝组织中的表达,探讨HCC的发生机制。方法免疫组织化学染色SP法检测31例HCCIGF-ⅠR表达。结果正常肝组织胞膜、胞质均显淡黄色,分布均匀。在HCC中,IGF-ⅠR呈异常高表达90·3%(28/31),以膜及胞质混合型表达为主,其表达与肿瘤的大小、病理分级无显著性差异(P>0·05),在AFP、HBsAg、HCV抗原阴、阳性组间无显著性差异(P>0·05)。肝内多发结节或单发结节合并门脉癌栓的HCC,IGF-ⅠR高表达与单结节HCC组间有显著性差异(P<0·05)。HCC癌与癌旁组织均显著高于正常肝组织。结论IGF-ⅠR高表达可能与HCC的侵袭性行为有关。 Objective To study the expression of Insulin-like growth factor Ⅰ receptor (IGF- ⅠR) in Hepatocelhtlar carcinoma (HCC), pericancerous and normal tissues in order to provide the theoretical and experimental basis for the pathogenesis of HCC. Methods 31 cases of HCC were detected by using immunohistochemical SP method. Results The expression of IGF-ⅠR was stained both in cytoplasm and membrane of normal liver cells. 28 of 31 HCC were stained abnormally high expression of IGF-ⅠR protein (90.3%) as a mixture of both membrane and cytoplasm staining. The expression of IGF-ⅠR protein were not significant difference with the size of tumor and pathological grade (P 〉 0.05). There is also no correlation between the positive and negative group of AFP, HBsAg and HCV (P 〉 0.05), but there were significant difference between multinodullar or single nodular with portal vein cancerous thrombus and single nodular HCC (P 〈 0.05). meanwhile, the expression of IGF-ⅠR in cancerous and peficancerous tissues of HCC were higher than normal tissues. Conclusion The higher expression of IGF-ⅠR may be associated with invasive behaviour of HCC and have a sighificanee for predicting the malignant change of bepatocytes.
