摘要 目的测定妊高征患者与正常妊娠孕妇血清及相应胎儿脐血血清中免疫细胞因子IFN-γ和IL-4水平,以探讨Th1/Th2型免疫细胞因子在妊高征发病中的作用。方法用ELISA法对38例妊高征患者(其中轻度妊高征11例,中度14例,重度13例)血清及相应胎儿脐血血清中IFN-γ和IL-4浓度进行测定,并选择同期16例正常孕妇作为对照组。结果①妊高征患者血清中IFN-γ含量显著高于对照组孕妇,并随病情加重呈增加趋势,重度妊高征患者增加极为显著(P<0.01);妊高征患者胎儿脐血血清IFN-γ含量亦显著高于对照组脐血含量,且随病情加重呈增高趋势,重度妊高征患者显著高于轻、中度者(P<0.01);②妊高征患者血清IL-4浓度显著低于正常妊娠者,随妊高征病情加重呈下降趋势,中、重度妊高征患者降低极为显著(P<0.01);妊高征患者胎儿脐血血清IL-4浓度显著高于对照组,随病情加重呈增加趋势,重度妊高征患者增高极为显著(P<0.01);③妊高征患者血清与相应胎儿脐血血清IFN-γ含量无明显差异(P>0.05),胎儿脐血血清IL-4含量显著高于相应母血血清含量(P<0.05);④妊高征患者血清IFN-γ增高与胎盘重量及新生儿体重呈显著负相关(r=-0.783,P<0.01;r=-0.742,P<0.01),母血血清IL-4含量的增高与胎盘重量及新生儿体重呈显著正相关(r=0.869,P<0.01;r=0.794,P<0.01)。结论Th1/Th2型免疫细胞因子的失衡可能是妊高征的发病机制之一。 Objective To investigate the variation of cytokines Interferon-У(IFN-У) and Interleukin-4(IL-4) in pregnancy induced hypertension (PIH) and normal pregnancy, and so to discuss the role of cytokine immunologic imbalance in the pathogenesis of PIH. Methods ELISA was used to measure the levels of IFN-У and IL-4 in maternal and umbilical serum of 38 PIH patients (study group) and 16 normal pregnancies (control group). The study group was further divided into three groups: mild PIH pations (11 cases), moderate PIH patients (14 cases) and severe PIH patients (13 cases). Results ①The levels of IFN-У in serum of PIH patients had a increasing trend as PIH degree elevated, in severe PIH, IFN-У levels were significantly higher than those of in normal pregnancy (P〈0.01). The levels of IFN-У in umbilical serum of PIH had a increasing trend as PIH degree elevated, too (P〈0.01); ②The levels of IL-4 in maternal serum of PIH patients had a decreasing trend as PIH degree elevated (P〈0.01). The levels of IL-4 in umbilical serum in PIH had a increasing trend as PIH degree (P〈0.01); ③There was no significant difference between the maternal and umbilical serum in the levels of IFN-У (P〉0.05), the levels of IL-4 in umbilical serum were significantly higher than those in maternal serum (P〈0.05); ④The levels of IFN-У in maternal serum had markedly negative correlation with the placental weight and newborn's birth weigh (r=-0.783, P〈0.01; r=-0.742, P〈0.01); The levels of IL-4 in maternal serum had markedly positive correlation with the lpacental weight and newborn's birth weight (r=0.869, P〈0.01; r=0.794, P〈0.01). Conclusion The levels of IFN-У and IL-4 are closely associated with PIH degree and may play an important role in the pathogenesis of PIH.
