冬眠降温治疗时机与继发性脑神经细胞损伤关系分析

摘要 目的探讨冬眠降温治疗时机选择对减少继发性脑神经细胞损伤的影响。方法筛选我院近10年来原发性脑损伤后不同时间开始冬眠降温治疗的病人192例进行回顾性分析,共分为4组,脑损伤后冬眠降温治疗的时机在2h内施行的共47例,~6h内52例,~8h内47例,〉8h以后施行的有46例。将所有病人伤后CT结果与伤后24h复查CT结果进行对照,测量每例病人两次CT检查的损伤灶体积,以损伤灶体积增加≤30%的例数及〉30%的例数为数据来源,进行统计学分析。结果各组进行比较,X^2=9.94.P〈0.025,2h与~6h实施冬眠降温治疗的病灶变化无统计学意义(X^2=1.89.P〉0.05),但〈2h与~8h组间、〈2h与〉8h组的病灶变化均有统计学意义(X^2=4.61,P〈0.05及X^2=9.24,P〈0.01)。结论脑损伤后越早实施冬眠降温治疗,继发性脑神经细胞损伤越轻。 Objective To investigate the efficacy of artificial hibernation and lower temperature on secondary brain injury. Methods Our hospital data of 192 patients of brain injury received artificial hibernation and lower temperature treatment were retrospectively analyzed. The patients were divided into four groups according to the different time (2 hours, ~6 hours, ~8 hours and after 8 hours) when the patients received the treatment after brain injury, and to compare the change of injury area volume between first injury and after 24 hours by brain CT. Results The brain CT injury volume changes was significantly different among 2 hours group, ~8 hours group and 〉8 hours group (P〈0.05 and P〈0.01). Conclusion The earlier applied the artificial hibernation and lower temperature treatment after brain injury, the less secondary brain injury.
