摘要
目的探讨可拆卸式金属熔附烤瓷(PFM)固定桥对基牙牙周微生物的影响。方法戴入可拆卸式PFM固定桥后,对基牙近缺隙侧(M)、远缺隙侧(D)、颊侧(B)、舌侧(L)龈下微生物在修复前及修复后1、3、6、9、12个月分别进行涂片和厌氧菌培养,检查球菌、杆菌及其他细菌的组成百分比,生化鉴定牙龈卟啉单胞菌、中间普菌、具核梭杆菌、伴放线放线杆菌、黏性放线菌。结果不同牙位在同一时期及同一牙位的不同部位在同一时期细菌检出百分比差异无统计学意义。修复后3个月内,较修复前细菌组成变化较大,3个月后,球、杆菌已恢复到常态指标。除伴放线放线杆菌始终未检测到外,牙龈卟啉单胞菌、中间普菌、具核梭杆菌均被检出,且随着时间延长呈逐渐上升趋势,12个月时较初检差异有统计学意义(P<0.01)。结论戴用PFM后,应在12个月内进行拆卸清洗,以维护牙周健康。本研究为可拆卸式PFM固定桥基牙的牙周防治提供了一定的理论基础。
Objective To study the effect of detachable porcelain fused to metal fixed bridges on periodontal microbial of the abutment teeth. Methods Periodontal bacterial floras of patients wearing detachable PFM fixed bridges were cultivated in anaerobic condition and analyzed. Results The percentage of the periodontal bacteria constitutes does not show a remarkable difference,at the different teeth and the same teeth but not the same part which are in the same period. In 3 months, the compose of bacterial has much more changes than that of before restored, but after 3 months, the compose of bacteria has come back to normal state guideline. The 3 bacterial ( P.g P.i F.n) have been checked out except Aa after 1,3,6,9,12 months respectively, the percentage has a gradual uptrend. The percentage of the second molars is much higher than the second premolars, which does not show a remarkable difference. Conclusion This investigation can provide theoretical basis for the prevention of periodontal diseases in patients wearing detachable PFM fixed bridges.
