摘要 目的检测正常妊娠与妊娠期高血压疾病患者不同孕周的血清胎盘生长因子(PLGF)水平,探讨母血PLGF水平是否可作为预测孕妇发生子痫前期的标记物。方法采用酶联免疫吸附法(ELISA)检测215例孕妇不同孕周的血清PLGF水平,其中正常妊娠组197例,妊娠期高血压疾病组18例。结果PLGF均呈峰形分泌,正常妊娠组在早孕期、孕20周、孕30周、孕40周时PLGF的均值分别为53.28pg/m l、323.63 pg/m l、596.78pg/m l、467.42 pg/m l,轻度子痫前期组为27.38 pg/m l、183.58 pg/m l、301.28 pg/m l、276.44 pg/m l,重度子痫前期组为22.52 pg/m l、95.72 pg/m l、217.92 pg/m l、148.56 pg/m l,轻度与重度组PLGF水平均显著低于正常妊娠组水平(P<0.01),并且随着PLGF水平的降低,妊娠期高血压疾病的病情亦逐渐加重。在孕早期PLGF≤32pg/m l时,预测子痫前期的敏感性为87.5%,特异性97.6%。在孕20周PLGF≤240pg/m l时,敏感性为75.0%,特异性为94.2%。结论PLGF对胎盘血管的形成有重要调节作用,PLGF的下降与妊娠期高血压疾病的发病相关,且PLGF的下降早于临床出现子痫前期的症状,可以作为预测子痫前期的一种标记物。 Objective To detect serum levels of placenta growth factor (PLGF) of different periods in normal pregnancy and patients with hypertensive disorder complicating pregnancy , and to explore whether or not PLGF is an early marker for the development of preeclampsia. Methods Serum samples were collected from 215 pregnant women ( normal group 197cases, hypertensive disorder group 18 cases) , PLGF levels were determined with enzyme - linked immunosorbent assay (ELISA). Results The secretion of PLGF likes mountain, the average of PLGF in normal group during the first trimester ,20th weeks,3Oth weeks and 40 th weeks before labor is 53.28pg/ml,323.63 pg/ml,596.78 pg/ml,467.42 pg/ml respectively, the average of PLGF in mild preeclampsia group during the first trimester ,20th weeks,3Oth weeks and 40 th weeks before labor is 27.38 pg/ml,183.58 pg/ml,301.28 pg/ml,276.44 pg/ml respectively, the average of PLGF in severe preeclampsia group during the first trimester ,20th weeks,3Oth weeks and 40 th weeks before labor is 22.52 pg/ml,95.72 pg/ml,217.92 pg/ml, 148.56 pg/ml respectively, the PLGF level of mild and severe group were significantly lower than that of the normal group ( P 〈0. 01 ) ,there was a trend that serum levels of PLGF decreased with the severity of preeclampsia. In the first trimester ,PLGF≤32pg/ml as early marker for the development of pleeclampsia,its sensitivity is 87.5% ,specific.ity is 97.6%, In 20th weeks PLGF ≤240pg/ml ,its sensitivity is 75.0%, specificity is 94.2%. Conclusion PLCF play important roles in placental angiogenesis throughout pregnancy. Decreased serum levels of PLGF is important during the development of hypertensive disorder complicating pregnancy, the decrease of PLGF is earlier than the symptom of preeclampsia, so it can be one of an early marker for the development of preeclampsia.
