摘要
目的探讨急性颅脑损伤后血清TNF-α,IL-1,IL-6,IL-8的含量变化及其临床意义。方法用放射免疫法检测50例急性颅脑损伤患者血清IL-1,IL-6,IL-8,TNF-α含量的变化,分析急性颅脑损伤分级与血清中IL-1,IL-6,IL-8,TNF-α含量之间的关系以及其变化趋势。结果重型颅脑损伤组在病程第1、3、7天三个时间点的血清TNF-α,IL-1,IL-6,IL-8的水平较轻、中型组明显升高(P<0.01),IL-6,IL-8早期即有升高,伤后3,7d升高明显,且有继续上升趋势;TNF-α,IL-1分别于伤后第3天达高峰,第7天有所下降,但仍明显高于正常人(P<0.01)。结论血清中TNF-α,IL-1,IL-6,IL-8水平在重型颅脑损伤中明显增高,与颅脑损伤程度呈正相关,并可能在继发性脑损害中起重要作用。
Objective To investigate the changes in the levels of serum tumor necrosis factor alpha (TNF-α) and interleukin - 1, 6, 8 (IL-1, IL-6 and IL-8) and their clinical significance in the patints with acute brain trauma. Methods The levels of serum TNF-α, IL-1,IL-6 and IL-8 were determind by radioimmunoassay in 50 patients with acute brain trauma. The relationship between the severity of brain injury and the levels of serum TNF-α,IL-1,IL-6 and IL-8 was analyzed in the patients with acute brain trauma, Results The levels of serum TNF-α,IL-1,IL-6 and IL-8 in the patients with severe traumatic brain. injury were significantly higher than that in the patients with mild and moderate injury (P〈0.01) on the 1st, 3rd and 7th day after the brain trauma. The levels of serum TNF-α, IL-1, IL-6 and IL-8 were significantly higher in the brain trauma group than that in the control group (P〈0.05). The levels of serum IL-6 and IL-8 were significantly higher on the 3rd and 7th day after the trauma than that on the 1 st day after the trauma. The levels of the serum TNF-α and IL-1 peaked on the 3rd day after the trauma, Although slightly dropped on the 7th day after the trauma, the levels of serum TNF-α and IL-1 were significantly higher in the trauma group than that in the normal group. Conclusions The levels of serum TNF-α,IL-1,IL-6 and IL-8 are significantly elevated in the patients with brain trauma, and are positively related with the severity of brain trauma. The inclease in the levels of serum TNF-α, IL-1, IL-6 and IL-8 after the brain trauma may play an important role in the brain injury secondary to the trauma.
