摘要
目的通过多普勒超声法对乳腺肿瘤内的血流信号进行分析,讨论动脉血流阻力指数=1.0及舒张末期反向血流的临床意义。方法对87例乳腺肿瘤患者检测分析其内部的血流信号,对动脉血流的RI值达到1.0或舒张末期反向血流的肿瘤对象进行回顾分析,并设对照组。结果对照组中测得的所有RI值均在1.0以下;病例组中RI=1.0的占31.0%(27/87),其中良性肿瘤的检出率是13.9%(5/36),恶性肿瘤则为43.1%(22/51),两者比较在统计学意义上有差异。舒张末期反向血流的占8.0%(7/87),其中良性肿瘤的检出率是2.8%(1/36),恶性肿瘤则为11.8%(6/51),呈现高的倾向。恶性肿瘤群中RI=1.0的检出率在统计学意义上没有显著差别。在良性肿瘤群中,6例导管内乳头状瘤中5例呈现检测出RI=1.0的动脉血流,占83.3%,和其它良性肿瘤相比在统计学意义上有明显差异。结论乳腺肿瘤内RI=1.0和舒张末期反向血流的动脉血流对乳腺恶性肿瘤及导管内乳头状瘤的诊断和鉴别诊断有指导价值。
Objective By means of Doppler ultrasound technique for analysis on the blood stream signals in breast tumors were undertaken, and the clinical significance of the arterial flow resistance index (RI)=1.0 and reversed flow in late-diastolic period were discussed. Methods Eighty-seven cases of breast tumor patients were studied through analysis on its internal blood stream signals with the Doppler ultrasonic examinations. We reviewed all the patients whose artery blood stream RI were=1.0 and reversed flow in late-diastolic period, and the controlled group was set up. Results In the controlled group, all their RI values were below 1.0; in the studied group, there were 31.0% (27/ 87)cases with the RI= 1.0, in which 13.9% (5/36)cases were picked out as benign tumors, meanwhile 43.1% (22/51 )were malignant. These two groups had obvious difference in statistic significance. There were 8.0%(7/87)cases with their RI=1.0 and reversed flow in late-diastolic period, in which 2.8%(1/36)cases were picked out as benign tumors, and 11.8%(6/51)were malignant. It showed the high tendency. There was no statistic remarkable difference for the picking out rate in the malignant tumor group with RI=1.0. But in the benign tumors group, 5 from 6 cases of intraductal papilloma patients (83.3%) were checked out that the RI of the arteries' blood streams were 1.0. It had obvious statistic difference compared with other benign tumors. Conclusion It has the instructive value for the diagnosis and identification for mammary malignant tumors and/or intraductal papillomas when the RI of arterial blood streams in mammary glands are=1.0 or reversed flow in late-diastolic period.
