摘要
目的:探讨低分子肝素(low molecular weight heparin,LMWH)对慢性肾功能衰竭(chronic renal failure,CRF)维持性血液透析(maintenance hemeodialysis,MHD)患者脂代谢的影响。方法:对58例慢性肾功能衰竭MHD患者的临床资料进行回顾性分析。结果:LMWH组的甘油三脂(TG)、总胆固醇(Tch)、低密度脂蛋白(LDL)、载脂蛋白B(Apo-B)在第9个月均低于UFH组(P均<0.05),第6个月无显著性差异(P>0.05),LMWH组的TG、Tch、LDL、Apo-B在第9个月低于第1个月(P<0.05);UFH组TG无显著性差异(P>0.05)、Tch、LDL、Apo-B高于第1个月(P<0.05)。结论:长期使用LMWH不加重血液透析患者脂代谢的紊乱,且可部分矫正已有的脂代谢的紊乱。
Objective：To Investigate the effect of low molecular weight heparin（LMWH）on lipid metabolism in the patients with chronic renal failure（CRF）maintenance hemeodialysis（MHD）. Methods：Triglycerides （TG）and cholesterol（CH）lipoproteins and apolipoprotein of 32 patients with maintenance hemeodialysis using unfractionated heparin（UFH） and 26 patients using LM- WH were investigated by retrospective analysis. Results： Compared with UFH group,a decrease of TG,Tch,LDL,and Apo-B was observed in LMWH group at 9 months （ P 〈 0. 05 ）. However there was no difference of above lipid levels between the two groups at 6 months（P〉0.05）. There was a decrease of TG,Tch,LDL,Apo-B at 9 months compared to those at 1 month （P〈 0.05） in LMWH group. There was no difference of TG between at 9 months and at 1 month（P〉 0.05）in UFH group. However, there were higher levies of Tch,LDL,Apo-at 9 months compared to those at 1 month in UFH group（P〈0.05）. Conclusion：In CHD patients, LMWH could improve metabolism of triglycerides and cholesterol lipoproteins and apolipoprotein.
