摘要
目的:探讨周小剂量卡铂联合低剂量氟尿嘧啶(5-FU)治疗65岁以上高龄晚期胃癌患者的临床疗效和毒副反应.方法:高龄晚期胃癌患者33例,随机分成两组:治疗组18例,采用静脉输注小剂量卡铂100mg/m2,每周1次,联合低剂量5-FU500mg/m2,每周1次,连续3周,休1周,重复3个周期;对照组15例,采用静脉输注卡铂300mg/m2第1天,联合5-FU500mg/m2第1~5天,每28d重复,重复3个周期.3个周期后进行临床评价.结果:治疗组18例,总有效率为50.00%,其中完全缓解0例,部分缓解9例;对照组15例,总有效率为46.67%,其中完全缓解0例,部分缓解7例(P>0.05).毒性反应:治疗组消化道Ⅱ°以上反应2例,Ⅰ°白细胞数下降1例,共16.67%;对照组Ⅱ°以上消化道反应7例,Ⅰ°以上白细胞数下降4例,共73.33%(P<0.05).结论:周小剂量卡铂联合低剂量5-FU是治疗高龄晚期胃癌患者的一种安全有效的化疗方案.
Objective: To evaluate the efficacy and toxicity of weekly low dosage carboplatin combined with fluorouracil(5- FU) in the treatment of old patients with advanced gastric cancer. Methods: Thirty three old patients with advanced gastric cancer were randomly divided into 2 groups. The treatment group (n= 18) received carboplatin 100 mg/m^2 and 5-FU 500 mg/m^2 once every week, iv,gtt,weeks 1,2,3. The second treatment cycle was given after rest for 1 week. At last 3 cycles were given. The control group (n=15) received carboplatin 300 mg/m^2 , and 5-FU 500 mg/m^2 on days 1-5, iv, gtt. One cycle was 28 days, At last 3 cycles were given. Results: In the treatment group, 9 patients had partial relief, the overall response rate was 50.00%. The major toxic effects were myelosuppression (1 patient, Ⅰ°) and gastrointestinal side-effects (2 patients,〉 Ⅱ°). The toxic effect rate was 16.67%. In the control group, 7 patients had partial relief, the overall response rate was 46.67% (P〉0.05). Seven patients had gastrointestinal side-effects (〉 Ⅱ°) and 4 patients suffered from myelosuppression (〉 Ⅰ°). The major toxic effect rate was 73.33% (P〈0.05). Conclusion: The efficacy of weekly low dosage of chemotherapy is similar to that of routine dosage. The toxic effect rate in the treatment group is lower than that in the control group. The therapy scheme may be a safe and effective treatment for the old patients with advanced gastric cancer.
