星形细胞瘤患者血清IGF-Ⅰ表达水平及其临床意义

摘要 目的:通过检测分析星形细胞瘤患者血清中胰岛素样生长因子-Ⅰ(IGF-Ⅰ)表达情况,探讨胰岛素样生长因子系统与星形细胞瘤发生的关系及其机制,并初步尝试在术前寻找一有助于星形细胞瘤诊断及预后评估的客观指标.方法:采用免疫放射分析(IRMA)测定30例星形细胞瘤(Ⅰ~Ⅱ级16例,Ⅲ~Ⅳ级14例)患者术前3d内、术后1周及30例正常人血清中IGF-Ⅰ的含量,并根据术后病理结果分组,进行对照分析.结果:低级别组与对照组、高级别组与对照组、低级别组与高级别组血清IGF-Ⅰ水平比较,其差异均存在显著性(P<0.05);手术前后星形细胞瘤病人血清IGF-Ⅰ表达水平的差异存在显著性(P<0.05).结论:①IGF-Ⅰ与星形细胞瘤的发生、发展有关,其血清表达水平与肿瘤恶性程度呈正相关;②临床上手术前检测IGF-Ⅰ水平对星形细胞瘤的诊断及预后评估有一定参考价值. Objective To investigate the serum expression of IGF - Ⅰ in patients with astrocytoma of different degrees of malignancy as well as the changes of levels after operative removal of the tumor. Methods Serum IGF - Ⅰ contents were measured with ⅠRMA in 16 patients with Grade Ⅰ ~ Ⅱ astrocytoma and 14 patients with Grade Ⅲ ~ Ⅳ astrocytoma both before and after operation as well as in 30 controls. Results The serum contents of IGF - Ⅰ in both groups of patients were significantly higher than those in controis (P 〈 0.05 ). The levels in GradeⅢ~ Ⅳ patients were significantly higher than those in Grade Ⅰ - Ⅱ patients ( P 〈 0.05 ). After operation, the levels dropped significantly (vs before operation, P 〈 0.05). Conclusion The serum contents of IGF - Ⅰ in patients with astrocytoma were positively correlated with the degree of malignancy. Post - operative decrease of IGF - Ⅰ contents was related to the decrease of tumor burden.
