摘要
目的:观察丙泊酚联合瑞芬太尼用于无痛人工流产术的安全性与可行性,并与丙泊酚联合氧化亚氮的方法进行比较.方法:选择100例需无痛人工流产的早期妊娠妇女,随机分为两组,丙泊酚联合瑞芬太尼组(PR组)与丙泊酚联合氧化亚氮组(PN组),每组50例.PR组:缓慢静脉推注瑞芬太尼1μg/kg后,继以丙泊酚1~1.5 mg/kg静脉推注;PN组:经面罩吸入50%氧气与50%氧化亚氮混合气体,直至临近手术结束,吸气开始时,静注丙泊酚1.5~2mg/kg.观察患者的循环、呼吸情况,麻醉镇痛效果,苏醒时间、离院时间,以及丙泊酚的总用量和术中的一些不良反应.结果:两组麻醉前、诱导后及术毕的血压、心率均无显著性差异(P>0.05),诱导后PR组的呼吸抑制发生率、氧饱和度与基础值及PN组同时点的值相比,显著下降(P<0.01),PR组的苏醒时间、离院时间及丙泊酚总用量均显著小于PN组(P<0.01),两组患者的镇痛优良率均为100%,无显著性差异(P>0.05).结论:丙泊酚联合瑞芬太尼用于无痛人工流产术安全可行.
Objective: To investigate the safety and feasibility of propofol and remifentanil used in induced abortion, and compared with propofol and nitrous oxide. Methods: One hundred early pregnant women were randomly divided into two groups (n = 50 in each ) : propofol and remifentanil group (PR group), propofol and nitrous oxide group( PN group). PR group was administrated remifentanil 1 1μg/kg and propofol 1 - 1.5 mg/kg;PN group: inhaled mixed 50% nitrous oxide-50% oxygen till operation nearly finished, and propofol 1.5 - 2 mg/kg was infused at the beginning of inhalation. The indications of circulation and respiration, the awake time and post-operation session in hospital, the total dosage of propofol were recorded, additionally, the analgesic effects and side effects were also observed. Results : There was no significantly difference in MAP and HR among premedication, post-induction and at the end of operation in two groups (P 〉 0. 05 ). After induction, a significant decrease in SpO2 and remarkable respiratory depression were founded in PR group( P 〈 0. 01 ). The awake time and post-operation session in hospital were significantly shorter in PR group than in PN group, the total dosage of propofol was also significantly less (P 〈 0. 01 ). No significant difference in analgesic effects and side effects were observed between the two groups. Conclusion: Propofol combined with remifentanil in induced abortion was safe and feasible.
