摘要
目的探讨肾脏移植术后HCMV间质性肺炎的临床特点及治疗对策。方法通过对23例肾脏移植术后HCMV间质性肺炎患者进行回顾性分析,总结其临床特点和治疗措施。结果23例患者治愈20例,死亡3例,治愈率为87%,且治愈患者移植肾脏的功能均正常。结论早期及时诊断,果断停用免疫抑制剂,适时适量应用激素,大剂量免疫球蛋白,提高患者被动免疫力,足剂量长疗程抗病毒治疗,及早进行高流量给氧或辅助呼吸等综合治疗是提高治愈率降低死亡率的关键。
Objective To investigate the clinical features and therapy of HCMV interstitial pneumonia after kidney transplantation. Methods A retrospective analysis of 23 patients after Kidney transplantation with HCMV interstitial pneumonia was undertaken to summarize its clinical features and therapy. Results 20 eases recovered and 3 eases died with a cure rate of 87% , and the grafts＇ function of the survived patients maintained normal level Conclusion The critical point which elevating recovery rate and decreasing death rate was included diagnosing HCMV Pneumonia promptly, stopping immunosupppressants, applying steroid and large dose immunoglobulin, antiviral therapy sufficiently, high flow oxygen and veatilator assisted respiration as soon as possible.
