摘要 目的:应用国产CAS-R-2型机器人系统(以下简称机器人),临床实施121例脑出血无框架立体定向手术。方法 机器人系统由智能机械臂、影像引导装置、三维定位软件等模块组成。该系统取代了传统的定向仪框架,通过识别贴附于患者头部的标志点.建立相应的三维坐标体系,并可自动标出靶点位置和穿刺轨迹。智能机械臂按照模拟轨迹固定方向,为术者提供准确导航和操作平台。结果:成功实现121例脑出血无框架脑立体定向穿刺置管引流手术,其中基底节区出血71例、脑叶出血19例,丘脑出血22例、小脑出血9例。结论:利用机器人辅助完成无框架立体定向手术,是一种方便快捷、定位准确、创伤小、安全有效等优点,应用前景广阔。 Objective: Frameless stereotactic operations were applied far the therapies of 121 cases of brain bemarrhage with domestic robot system CAS-R2 (for sbort: robot). Methods: This robot system consists of intellectual machinery arm, image-guiding device and 3 dimensional software. Instead of traditional stereotactic frame, the system established the 3D coordinate according to the marks added to patient s head. It showed the target position and the trace of puncture automalically. The intellectual arm could follow the trace and fix the direction, to give doctors the navigation and platform during operation. Results: 121 puncturations and drains of brain hemorrhage were successrully perlbrmed with frameless stereotactic operations, which included 71 basal nuclei haemorrhages, 19 brain lobe haemorrhages,22 thalamic hemorrhages, 9 cerebellar hemorrhages. Conclusion: It is convenient, quick, accurate microtrauma,saft and efficient for the robot system to assist the stercotactic frameless operations.There would be a wide application in neurosurgery.
