摘要
作者报告了用自体半侧腓骨髓腔内移植,梯形自动加压钢板固定治疗长骨干骨不连。生物力学实验证明,髓内植腓骨钢板固定组的最大扭矩、扭转强度等均明显高于不植腓骨组,固定力学效果佳。髓腔植入腓骨后,胫骨干表面及截骨面所承受的扭转应力明显低于不植腓骨组;钢板受力均匀,较不植腓骨组明显减少,设计符合等强度原理,可以减少应力集中和疲劳断裂,提高钢板的工作安全系数,且螺钉对钉孔周围骨质的挤压力显著减小。这对防止螺钉孔周围骨质坏死及螺钉松动均有一定作用。这些均为骨愈合过程创造了极有利的力学环境。实验证明了该植骨固定方法一次性充分完成长骨干不连的结构及功能重建的优越性和可行性。
Inordertoinvestigatethebiomechnicalpropertiesofplatefixationforthetreatmentofnon-unionoflongboneswithautogenoussemifibularintramedularygraftandtocomparetheplatefixa-tionwithoutfibulagraft,theauthorsconductedabiomechanicalstudy.Theresultsdemonstratedthattheplatefixationincorporatedwithintramedularysemifibulagraftexhibitedsignificantlyhigherbiome-chanicalproperties,suchastheultimatetorsinoalmoment,thestrengthandtheelasticmodulusetc.;however,thetorsionalstressattheosteotomiessites,tibiaandplatesurfacewaslower.Thestressoftheplatewithintramedularyfibulagraftiswel-distributed,sothatitsconcentrationandthechanceofbreakageoftheplatearereduced.Thecompressiveforceoftheosseoustissuesurroundingthescrewsisremarkablylowerthanthatfixedwithoutthefibulagraft.Platefixationwiththefibulagraftcertainlyplayedtheroletopreventlocalbonenecrosisandthelooseningofthescrewsandprovidedanexcelentmechanicalandbiologicalenvironmentforthehealingofnon-union.
