中心静脉压监测下加压快速扩容抢救失血性休克的临床观察 被引量:6

Clinical application of pressured rapid volume expansion in the treatment of hemorrhagic shock under the monitoring of central venous pressure

摘要 目的探讨中心静脉压(CVP)监测下,采用加压袋装液体快速扩容法抢救失血性休克的临床运用及效果。方法40例失血性休克患者随机分为研究组(Ⅰ组n=20)和对照组(Ⅱ组n=20),Ⅰ组在CVP监测下,采用血压计袖带加压软性袋装液体快速静脉输注法;Ⅱ组用传统输液方式处理。结果Ⅰ组病例在30m in内平均输入量为(1271±170)m l,在围术期平均总输入量为(5311±410)m l;Ⅱ组分别为(721±140)m l与(3273±330)m lP<0.01,SBP、DBP、CVP、SpO2指标恢复,排尿恢复时间,治愈率明显优于Ⅱ组;死亡率下降。结论在CVP监测下,实施加压快速扩容法是救治失血性休克的重要措施,可迅速恢复病人有效循环血量,维持重要器官组织细胞的灌注,提高抢救成功率。 Objective To explore the clinical effect of pressured rapid volume expansion in the treatment of hemorrhagic shock under the monitoring of central venous pressure (CVP). Methods Forty patients with hemorrhagic shock were randomly divided into two groups: the study group (group Ⅰ , n=20 ) and the control group (group Ⅱ , n=20). In group Ⅰ , the patients were treated with rapid intravenous infusion by pressured soft fluid in bags under the CVP, and the patients received traditional transfusion method in group Ⅱ. Restilts The average amount of transfusion in 30 minutes and the total amount in perioperative period were ( 1271±170) ml and (5311±410) ml in group I respectively, and in group Ⅱ , the results were (721±140) ml and ( 3273±330) ml respectively. The recovery of systolic blood pressure(SBP) , diastolic blood pressure( DBP), central venous pressure(CVP) and pulse oxygen saturation( SpO2 ) , the time of recovered urination and cure rate in group Ⅰ were significantly better than those in group Ⅱ. The mortality reduced in the study group. Condusion With the monitoring of CVP, the pressured rapid volume expansion is an effective treatment for hemorrhagic shock, With the pressured rapid volume expansion, the circulating blood volume of patients can be recovered rapidly and the perfusion of important organs, tissues and ceils be maintained.
